Friday, 02/05/16:
The track rig started shift set up on DXT2177. Grout cart 1 set up next to the rig and began grouting the second stage of the annulus at 0737. After topping off, the stage took 110 gallons of grout. Cascade removed the casing and moved off the hole. The interface was grouted from 1346 to 1427, using 64 gallons. A piezometer was over the threshold limit defined by USACE, so grouting stopped. The interface was close to closing off before the threshold was reached.

Field Notes: 03/10 - The track rig set up on hole UXP2182 from 1202-1530 advancing to 135 feet before lightening ended the day at 1530.
03/11 - The track rig finished hole UXP2182 from 707-834. The hole hit apparent top of rock at 144.5 feet in the beech creek limestone. During cleanout of the hole clay filled void was encountered from 144.7 to 149 feet filled with saturated, fine, soft, clay soil and weathered limestone. The beech creek was then again encountered from 149-150 fbgs with an underlying clay seam. From 150-154 small limestone gravels and Elwren Shale were encountered at the bottom of the run. This feature is unlikely related to a boulder when DXP2182 is considered which hit 12 feet of limestone from 139-151 feet. The sleeveport pipe was inserted and the barrier bag was set at 149 fbgs and grouted from 858-922 with a grout take of 7 gallons at 42psi. The 1st annular space was grouted from 926-958 taking 135 gallons at 64psi. No grout was identified in the tailwater as expected and the hole refused taking approximately 50 more gallons of grout over the theoretical volume.
03/12 - UXP2182 had the stage 2 annulus grouted from 1005-1042 taking 100 gallons of grout at gravity pressure.

Field Notes: 12/16 - The track mounted sonic rig moved to DXS2192. Began drilling at 1042. Drilled through the embankment without issues.
The driller thought he had encountered rock at 133 feet, so he ended his run from 125 feet at 133 feet. The run from 133 to 141 feet was drilled from 1343 to 1352. The sample had clay at the top of the run down to the top of rock. The top of rock was at 138 feet, and the bottom of the hole was at 141 feet. Cascade inserted the sleeveport PVC. ACT then moved grout cart 19-10-07 (operational again) over to the hole. The “bag-mix” was batched in the portable mixer near the hole, and grouting the bag began at 1500, using 3 gallons. The first stage of the annular space was grouted from 1542 to 1609, taking 96 gallons – Cascade removed 60 feet of casing. Cascade stayed on the hole so they can grout the second stage in the morning. ACT set up the grout cart to grout the interface. Began grouting at 1640. The intellisystem operator kept thinking there must be grout coming up in the casing because the pressure was not building. The operator had the laborers on the platform continue checking the casing for grout. After several batches of A-mix, we switched to B-mix. After two medium sized batches of B-mix, ACT let the hole rest for night before continuing to grout in the morning. Grouting completed at 1855 with 591 gallons of A-mix and B-mix. We were flowing at 7.5 gpm and had reached ~52psi pressure when the last batch of B-mix ran out at the end of shift.
**Note: ACT requested to switch to B-mix, and I agreed with the request. Also note, ACT recommended we stop grouting after the second batch of B-mix, and I agreed. Both requests can be seen as direction from the Corps due to the Specification requiring government direction for both changing a mix and to stop grouting without closure.

12/18 - The Core rig set up on DXS2192 and drilled from 141 to 156 feet this morning. Drilled from 0748 to 0948. The core rig flushed the hole. The Televiewer survey was run on DXS2192. Grouting of first 15 feet of rock for DXS2192: Grouting began at 1229. After running A-mix grout for a while, the pressure was not building fast and was not approaching the target of 90 psi. At 1245, I directed ACT to batch 3 A-mixes, 2 B-mixes, 2 C-mixes and so on. I was notified grout was seen in the river at 1300. At 1400, the flow was at 7.5 gpm and the pressure was at 52 psi. ACT began the first batch of B-mix. After meeting with other Corps employees it was decided to increase the mix changes to one batch each. When I got back into the grout cart, the second B-mix was just being made. Then ACT batched one C-mix followed by one D-mix. At 1600 the pressure was approximately 70 psi and the flow was at 5.5 gpm (ACT had just run down the batch of D-mix) and they stopped grouting to get the sanded mix batched.
They had to set up the sand mix portable grout mixer, get it to run, and mix up the first batch of E-mix (the sanded mix, which is D-mix plus sand). The grout cart operator had been told to remove the packer from the hole just in case the grout set up. When the crew was ready, the packer was lowered back in the hole. The packer had been set at 141 feet earlier in the day, but the packer could only be lowered to 139 feet this time. Grouting resumed at 1725. The pressure was increasing much faster than it had earlier in the day, but the flow had to be significantly decreased or completely stopped several times to make new batches of E-mix. At 1859 target pressure (90 psi) was reached, and the flow was at 7.5 gpm. At approximately 1930, a QC told me we had pumped just over 400 gallons of the sanded mix into the hole (according to Spec, ACT is required to inform the Corps when the sanded mix is at approximately 500 gallons – so the Corps can decide if they want to continue grouting or to let the hole set to resume grouting at a later date). We agreed it was best to stop, and we decided to use the rest of the grout in the mixer. At 2001, we were out of grout – the flow was at 6.2 gpm at target pressure. The total grout amount pumped today was 1913.9 gallons (A-mix through E-mix combined).

Field Notes Monday, 03/14/16:*
The track rig set up on hole UXS2192 from 923-1310. There was an intermixing of soft clay infill with stiff clay above the top of rock. The largest area of clay infilling was approximately 2 feet thick and occurred above the beech creek limestone between 143 and 145 fbgs. The material had 0 shear strength and had a consistency of pudding to creamy peanut butter. Below this infill was weathered limestone overlying some slightly more competent limestone. Two 8 inch thick clay seams occurred just below the top of the beech creek near 146 and again at 147.5. The clay seams were very soft and plastic and also had 0 shear strength. Some of the beech creek limestone showed some solutioning and there appeared to be some vertical joints in the clay seams. From 148- 151 the drill encountered 1 additional foot of beech creek limestone and 2 feet of the Elwren Shale. The top of rock was called at 145 and the bottom of hole was 151. The sleeveport pipe was inserted and the barrier bag was grouted from 1443-1515 with a grout take of 8 gallons at 36psi. The 1st annular space was grouted from 1522-1540 and took 100 gallons at 64psi

Gray grout and clay was encountered overlaying a thin limestone cap. Limestone was noted from 131-133 fbgs. A tool drop was noted from 133 to 136 in the clay infill. It is assumed that very little of this material was recovered as part of that run and the infill was recovered as part of the next run with very soft, wet, clay infill present from 133-139 fbgs. The clay infill had a creamy texture ranging from pudding to peanut butter. Bag samples were taken at 131-133, 136.5-137.5, 137.5-139. 1 feet. Competent limestone was encountered below the void from 139 of 141 fbgs.

Water loss between 133-135’
Grout Take 136-151’
Field Notes: 02/20 - ACT began downstage grouting on hole DXS2232 in the zone where they had water loss on Friday. They began injecting A mix at 0730 and almost immediately saw a reaction in PZ35. They injected 3 medium batches of A-mix (409 gallons, 0730-0844) followed by 1 medium B-Mix (139 gallons, 0844-0906). Pressure was slowly building in the grout hole and the pressure in PZ35 continued to rise. The GOV directed ACT to switch to D-mix to try to seal the hole while continuing to monitor the pressure in PZ-35. Grouting was stopped at 0945 when PZ reached a level of approximately 554 after injecting approximately 297 gallons of D-Mix. The pressure in PZ-35 began to drop almost immediately upon stopping the grouting operation. The total grout injected was approximately 845 gallons. The maximum pressure that was achieved was 52.6 psi and the target pressure was 65.2 psi. ACT will re-setup on the hole Monday to try to close the hole.
02/22 - Grouted hole DXS2232 again for downstage grouting. Packer was set at a depth of 115' and target pressure was 69 psi.
Grouting started at 1202-1325 with A-Mix Grout pumping approximately 371 gallons into the hole. The injection pressure was at 69 psi so B-Mix grout started at 1325 and took 249 gallons of grout. At 1527, C-Mix was started to close up the hole. 122 gallons CF of C-Mix grout was pumped into hole to reach refusal. The total grout injected today was 742 gallons. The total gallons to reach closure was 1586 gallons.
06/06 - DXS2232 0915-1138, Stg. 3 from 136-151’ took 865 gal. total (557 gal. of A-mix and 308 gal. of B-mix) the hole did not refuse and was flowing 7 g/m at 79 psi. when operations ceased due a 40’ jump in PZ40
06/07 – Grouted DXS2232 0752-0948; Stg. 2 from 136-151’ took 158 gal. at 81 psi

DXS2252, Interface Grouting, 45 gal.
1011-1023
Field Notes Wednesday, 12/16/15:
The track mounted sonic rig started shift set up on DXS2252 with hole to full depth. Grout cart 19-10-05 (only operational grout cart through the morning), set up to begin grouting the bag. The bag was grouted starting at 0750, and took 7.7 gallons. The first stage of the annular space was completed at 0846, and took 74.9 gallons. Cascade removed casing and moved off the hole. The interface was grouted from 1008 to 1024 (when grout came to the surface of the annular space). It took 44.7 gallons, but ACT flushed the annular space down to 35 feet almost immediately after grouting.
The track mounted sonic rig and grout cart moved to DXP2122. The second stage of annular space was grouted from 0926 to 1051, and took 101 gallons. Cascade removed the casing and moved off the hole.

• Grouted DXS2252 1003-1538; Stg. 3 from 140-155’ and took a total of 1218 gal. (489 gal.
of A-mix, 155 gal. of B-mix, 387 gal. of C-mix, and 187 of D-mix), the hole did not refuse and was still accepting ~5 g/min. at 90 psi when operations ceased. The water cart washed the hole upon termination and lost return fluid immediately. The operation was ceased. See bottom of this entry for special PZ details.
• Manual Readings were required for PZ40 during grouting of DXS2252 because the pressure transducer in the borehole flatlines at a peizometric pressure of 500 feet.
Manual readings were taken on the following intervals included below. It should be noted that instantaneous reactions were observed between the grouting operation and the PZ, including the gate operation at 1200 hours. It is estimated that the PZ has the ability to jump 25-30 feet in the matter of a minutes. At 14:10 grouting operations were terminated because the piezometric pressure reached to within 10 feet of the ground surface. 45 minutes later grouting was resumed at a lower pressure and grout was accepted until Plant 1 ran out of cement.
