A brief site description and an overview of the contractor’s work performed on the project as it relates to the scope of work and the elements identified under relevant experience.

Identify project challenges, risks and unknowns impacting the project and describe how your company approached and addressed these challenges, risks and unknowns to minimize impacts to the scheduled completion of the project. Include important project characteristics such as site restrictions, cost and schedule constraints, customer and stakeholder constraints, health and safety, and other unique considerations used to mitigate the project challenges, risks, and unknowns associated with the project.

Include a narrative on how experience submitted provides technical knowledge and capability to perform the specific work item of this solicitation.

Projects that do not contain all required information may not be evaluated as one of the minimum required projects. This may result in a lower confidence rating.

A sample PPQ is included in the solicitation. USACE requests the Offeror’s client completes this questionnaire and submits it directly back to the Offeror. Ensure correct phone numbers and email addresses are provided for the client point of contact.

Completed PPQs should be submitted with your proposal.

If the Offeror is unable to obtain a completed PPQ from a client for a project before proposal closing date, the Offeror should complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project(s). Offerors should follow- up with clients/references to ensure timely submittal of questionnaires. Offerors must provide all PPQs for consideration with their proposal.

However, this does not preclude the Government from using previously submitted PPQ information in the past performance evaluation. Offerors may submit CPARS in lieu of past performance references, if applicable.

In addition to the above, the Government is required to use CPARS as a source for past performance, when available. If not available and when necessary, the Government reserves the right to check any or all cited references to verify supplied information and to assess owner and engineer satisfaction regarding qualifications and past performance. Other sources may include, but are not limited to, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the Offeror. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate, and complete past performance information rests with the Offeror. The Government may not contact all of the identified client references/points of contact. This assessment is based on the Offeror’s record of relevant and recent past performance information that pertains to the specifications outlined in the solicitation requirements. If the firm is divided into severable segments (e.g., division, group, or unit) the Government will evaluate only the past performance of those segments of the firm(s) that will perform the work.

Information gathered from these sources, if any, must have evidence of at least “Satisfactory” performance in the past fifteen (15) years.

NOTE: For purposes of evaluating past performance, the Prime Contractor is defined as the contractor identified in Block 14 of the Standard Form 1442. Projects performed by contractors other than the offeror, including, but not limited to, teaming partners, subcontractors, sister or parent companies, and affiliates will not be evaluated for past performance, unless those other contractors are part of a joint venture offeror as demonstrated by a signed joint venture agreement, or are a planned sub-contractor for this project as demonstrated by a signed Subcontractor Letter of Commitment (Appendix D). If more than one contractor is listed in Block 14, then a signed joint venture agreement must be submitted with the proposal and the joint venture shall be registered as such in the System for Award Management (SAM). However, each party of the Joint Venture (JV) must submit their own Unique Entity Identifier (formerly DUNS) number with the JV proposal. If the Offeror represents the combining of two or more companies as a joint venture for the purpose of this Request For Proposal, each company in the joint venture may submit project examples, but the total submitted by the joint venture will not exceed five (5).

VOLUME II – SMALL BUSINESS PARTICIPATION

A Factor 3 – Small Business Participation Commitment B Subcontracting Plan

TAB A: Factor 3 – Small Business Participation Commitment

All Offerors (both large and small businesses) are required to complete a Small Business Participation Commitment Document (Appendix C), in accordance with DFARS 215.304. Offerors should propose the level of participation of small businesses (as a small business prime, joint venture, teaming arrangement, and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in the evaluation of this area (in Section 00 22 16) and provide substantiating documentation to demonstrate how you will meet these requirements.

Large business contractors may achieve the small business participation goals through subcontracting to small businesses. Small business contractors may achieve small business participation goals through their own performance/participation as a prime and also through a joint venture, teaming arrangement, and subcontracting to other small businesses in order to achieve the small business participation goals.

The Offeror shall articulate within their Small Business Participation Commitment Document (SBPCD) how they intend to meet the small business objectives, requirements, and participation goals. Nothing precludes an Offeror from further demonstrating their extent of commitment to use small businesses beyond what has been required by the subject solicitation. Offerors are encouraged to submit proof of awards, accolades, etc., received for their current and/or past support of small businesses. Examples include clear pictures of award(s) received in support of small business, successful Small Business Administration Surveillance Review findings, CPARS ratings that address small business utilization, testimonials letter of recommendation, etc. Discuss and submit proof of any specific outreach efforts (virtual or in person) performed in support of this requirement (e.g. holding internal or external meetings with potential small business subcontractors, ads, any follow up correspondence with small businesses as a result of conference attendance or hosting outreach events) that demonstrates the Offeror’s effort to identify small business subcontractors specifically in support of this requirement. For example, a roster or correspondence specifically generated to demonstrate how the Offeror sought out small businesses to support them on this requirement at a Small Business Conference, attendance sheet, email correspondence, etc., proof of any other pertinent information that demonstrates the Offeror’s current and/or past efforts to support small businesses.

The Source Selection Evaluation Board (SSEB) will not look to obtain information in support of an Offeror’s Small Business Participation Plan (SBPP) from other binders/binders or within the Subcontracting Plan (when applicable). The SBPP (IAW DFARS PGI 215.304) and the subcontracting plan (IAW DFARS PGI 219.705-

4) are two distinct separate documents with different elements and should not be combined for submission. Offerors submitting SBPPs for this project should limit submissions to data essential for the evaluation of plans. Material submitted in support of the SBPP shall be organized sequentially under applicable tabs and all pages must be numbered to facilitate evaluation.

TAB B: Subcontracting Plan

Separate from a SBPCD, other than U.S. Small Business Offerors must also submit a subcontracting plan meeting the requirements of DFARS PGI 219.705-4.

Subcontracting Plans shall reflect and be consistent with the commitments offered in the SBPCD. The dollars should correlate directly to the percentage of subcontracted dollars in the Small Business Subcontracting Plan. If an Offeror specifically identifies small business subcontractors to support the subject requirement within its SBPCD those small businesses shall be listed in any subcontracting plan submitted pursuant to DFARS PGI 219.705-4.

The Small Business Subcontracting Plan is not a requirement for evaluation in source selection, but rather a requirement for award to an Other than Small Business Only. The apparently successful Offeror's Small Business Subcontracting Plan shall be assessed IAW DFARS PGI 219.705-4 and must be determined as “acceptable” by the Contracting Officer in the event of an award. A Subcontracting Plan deemed as “unacceptable” may preclude an Offeror from being eligible for contract award. Although submission of a subcontracting plan is required, subcontracting plans will only be evaluated for the Offeror(s) deemed to be apparently successful prior to award. The Small Business Subcontracting Plan will not be included in the page count.

The DoD Checklist for Reviewing Subcontracting Plans is provided for your information (Appendix E) as it is what will be used to evaluate the Subcontracting Plan for approval.

VOLUME III – PRICE PROPOSAL & ADMINISTRATIVE SUBMISSION

Page Limits. There are no page limits set for the price proposal. However, limit your response to information required by this solicitation. Excess information will not be considered in the Government’s evaluation.
