Attachment 6 Past Performance Cover Letter and Questionnaire

Command Information Management System (CIMS) III Past-Performance Questionnaire Instructions:

1) Offeror: Offerors interested in bidding on the CIMS III Solicitation # W91260-17-R-0001 shall fill out questionnaire sections 1 & 2 for each past performance citation being submitted in the Past Performance Volume of their proposal. Offeror shall forward the attached cover letter and questionnaire to the referenced point of contact (POC) and request the POC complete and forward prior by the stated suspense date to the Contracting Office POC’s. Other than copying to your letter head and completing the fill-in’s, the cover letter (POC’s etc.), shall not be change by the offeror in any way.

It is the Offerors responsibility to ensure past performance citations are received from each reference.

1. We are currently responding to request for proposal # W91260-17-R-0001 issued by the Army Contracting Command – Redstone Arsenal (ACC-RSA) Colorado Springs Contracting Office for the Command Information Management System III (CIMS III) contract.

2. The major consideration in proposal evaluation is verification of our past performance on contracts which reflect our ability to perform on the proposed effort. This acquisition requires that we provide a past performance questionnaire to you since I believe you have first-hand experience with our performance on the contract, identified in the subject line.

3. It is extremely important to us that you please take a few moments to complete sections 3 and 4 of the attached questionnaire. Once completed, the detailed information contained in the questionnaire is considered Source Selection Sensitive and will NOT be released to us, the Offeror. Return responses directly to the contracting office as soon as possible, but not later than TBD by encrypted e-mail:

SECTION 2: DESCRIPTION OF CONTRACT EFFORTS

The contractor cited in Section 1 is submitting this questionnaire due to one or more similarities (its relevance) with the planned Command Information Management System III (CIMS III) requirement they are planning to bid on. The CIMS III contract provides U.S. Army Space and Missile Defense Command/Army Forces Strategic Command (USASMDC/ARSTRAT) with Information Technology (IT) Operations and Maintenance. The CIMS is the overall command enterprise and support architecture comprising the primary functional IT entities required to support the command’s sustaining operations. The contractor shall support all functional areas to include, but not limited to: Program Management, Help Desk, Cybersecurity, Network Administration, Systems Administration, Software Development, Systems Integration and Maintenance, and Infrastructure and Cabling for an Indefinite Quantity Indefinite Quality (IDIQ) contract with hybrid Contract Line Item Number (CLIN) types to include firm-fixed price, labor hour, and cost-plus fixed-fee.

The contractor cited in Section 1 believes the contract cited in this questionnaire is similar in one or more ways to the CIMS III requirements; therefore suitable to use as past performance evaluation for one or more of the following reasons:
