OCCUPATIONAL HEALTH / INDUSTRIAL HYGIENE (OH/IH)
PERFORMANCE WORK STATEMENT (PWS)
(INDIRECT)
1. OBJECTIVE: The contractor shall maintain and operate an occupational health and safety program IAW applicable Federal, State and local regulations.
2. REQUIREMENTS: The Occupational Health and Safety Administration (OSHA) exposure standards permissible exposure limit (PEL) shall be used except where the American Conference of Governmental Industrial Hygienists (ACGIH) threshold limit values (TLVs) are more stringent. Exposure standards and criteria shall be used to determine the need for medical monitoring, occupational health screening, and the need for protective equipment and/or reducing the exposure through engineering controls.

2.1 The contractor shall evaluate workplace hazards related to the safety and health of its employees. The contractor shall keep a hazard abatement log as described in the Safety PWS paragraph 4.5. The hazard abatement log shall include, at a minimum the location of the hazard, the potential for injury or illness, the method of control or plan to mitigate the risk, and the estimated abatement date.
2.2 When the Contractor conducts industrial hygiene monitoring, such as indoor air quality, noise sampling, confined space entry permits, and other statutorily based sampling, a written copy of the results shall be forwarded to the Administrative Contracting Officer (ACO). The contractor is not required to provide individual personally identifiable information or personal health related information to the ACO (CDRL A001).
2.3 The Contractor shall provide the ACO with access to its repository for chemical inventory within 90 days of contract award. The contractor shall notify the ACO within seven calendar days of any significant change in the chemical inventory, to include the addition of a hazardous material or compound which includes at least one hazardous material listed in Table Z-1, Z-2, or Z-3 of 29 CFR 1910.1000, or any material which has been deemed hazardous by the National Institute for Occupational Safety and Health or the chemical manufacturer.
2.4 The Contractor shall provide the ACO with access to its repository to allow for viewing of the most current Material Safety Data Sheets (MSDS) for the chemicals used in work processes or stored at the facility. Proprietary information shall be handled in accordance with established OSHA standards.
2.5 The Contractor shall provide a copy of its written respiratory protection program to the ACO (CDRL A002). The contractor shall ensure that personnel who are required to wear respirators have completed a medical evaluation, fit testing, and training in accordance with 29 CFR 1910.134. A memorandum attesting to the fact that the contractor is in compliance with the respiratory protection standard shall be provided to the ACO.
2.6 The Contractor shall record and provide a copy to the ACO of all cases of occupational hearing loss and other work related medical conditions in the copy of its OSHA Form 300 as required by 29 CFR 1904, and as described in the Safety PWS paragraph 2.2.2. The Contractor shall provide personal protective equipment for its employees in accordance with 29 CFR 1910.132, 29 CFR
1910.133, 29 CFR 1910.134, 29 CFR 1910.135, 29 CFR 1910.136, 29 CFR
1910.137, and 29 CFR 1910.138. The workplace hazard assessment (required by 29 CFR 1910.132(d)(2)) shall be made available to the Government Staff as requested and copies may be made as desired. The contractor shall ensure that employees are properly fitted and trained to don, use, doff, and maintain personal protective equipment. Training documentation shall be made available to the Government Staff as requested and copies may be made as desired.
2.7 The Contractor shall prohibit the consumption of food and beverages in areas exposed to toxic materials in accordance with 29 CFR 1910.141(g)(2). A copy of the Contractor’s policy which prohibits the practice shall be provided to the ACO
(CDRL A003).
2.8 The Contractor shall provide a copy of its Permit Required Confined Space Program to the ACO (CDRL A004). The program shall be in accordance with 29 CFR 1910.146. If the Contractor’s employees enter permit required confined spaces, a written statement of certification is required to be furnished to the ACO. Additionally, the Contractor shall label permit required confined spaces in accordance with 29 CFR 1910.146(c)(2), and maintain the labeling throughout the contract period.
3. OCCUPATIONAL MEDICINE:
3.1 The contractor shall maintain and provide employee records in accordance with 29 CFR 1910.1020.
