About this file
This is an instructional guide document from the Illinois Department of Public Health (IDPH) detailing the process for registering veteran patients in the state's medical cannabis program. The document outlines the application procedures for veterans with a DD-214 discharge certificate seeking to participate in the Medical Cannabis Patient registry. The registration process requires veterans to complete mandatory fields marked with red asterisks, review qualifying conditions from the state's List of Debilitating Conditions, and select from two application pathways: submission of Veteran Administrative Health Records or Health Care Professional Certification. Veterans may upload multiple supporting documents through the portal and must proceed through a Proof of Identity verification section to complete their application.
The document specifies that applications submitted through Veteran Administrative Health Records will maintain an "Awaiting Review" status until documentation is processed, while applications utilizing Health Care Professional Certification will be activated immediately upon submission. When selecting the Health Care Professional Certification pathway, the HCP determines specific medical information including Terminal Illness status, Lifelong Condition designation, and Allotment Limits for the veteran applicant. The guide provides navigation instructions for portal users and references external resources for condition eligibility criteria and identity verification requirements through links to the IDPH medical cannabis program website.
