About this file
This document is a procedural guide issued by the Illinois Department of Public Health (IDPH) regarding the extension process for medical cannabis patient certificates under the Medical Cannabis Pilot Program (MCPP). The guide outlines the application workflow for patients whose certificates are approaching expiration and have achieved "Pending Extension" status, which occurs automatically 60 days prior to certificate expiration. Patients may select either a one-year or two-year extension term through the online application portal. The extension process eliminates the requirement for a new healthcare professional certificate, as the current certificate remains valid for three years. The procedural steps include navigating the patient information page, caregiver information section (with options to add new caregivers), and application fee payment page, with system prompts guiding users through each stage of the extension application.
The document specifies that patients must complete their selected term before becoming eligible for extension and must pay applicable extension fees. The extension application utilizes an online portal with auto-populated patient information to streamline the process. No specific contract award dates, bidding requirements, pricing structures, or renewal terms are detailed in this procedural document, as it serves as an instructional guide rather than a formal solicitation or contract award announcement.

Text version
The extension of the cannabis certificate is only applicable to MCPP patients with an active card in "Pending Extension" Status. Sixty days before expiration, the certificate's status will update to Pending Extension.
Once the patient has chosen either a one-year or two-year option while filling out the application, upon completing the selected term, they will be allowed to extend their medical cannabis card. A new health care professional (HCP) certificate will not be required for extensions as the current certificate remains valid for three years.
