NOTICES AND SPECIAL INSTRUCTIONS TO ALL CONTRACTOR SITE VISIT PARTICIPANTS
1. In accordance with Executive Order 13991, “Protecting the Federal Workforce and Requiring Mask-Wearing,”
January 20, 2021, and Office of Management and Budget Memorandum, “COVID-19 Safe Federal Workplace:
Agency Model Safety Principles,” January 24, 2021, all individuals on military installations, as defined below, and all individuals performing official duties on behalf of the Department from any location other than the individual's home, including outdoor shared spaces, will wear masks in accordance with the most current Centers for Disease Control and Prevention (CDC) guidelines. Individuals must wear masks continuously while on military installations except: (1) when an individual is alone in an office with floor-to-ceiling walls with a closed door; (2) for brief periods of time when eating and drinking while maintaining distancing in accordance with CDC guidelines and instructions from commanders and supervisors; (3) when the mask is required to be lowered briefly for identification or security purposes; and (4) when necessary to reasonably accommodate an individual with·a disability.
Individuals must consistently wear a mask that covers the nose and mouth and that comports with all current guidance from the CDC and the Occupational Safety and Health Administration. Locations where masks must be worn include any common areas or shared workspaces (including open floorplan office spaces, cubicle embankments, and conference rooms) and in outdoor shared spaces. Masks recommended by the CDC include non-medical disposable masks, masks made with breathable fabric (such as cotton), masks made with tightly woven fabric (i.e., fabrics that do not let light pass through when held up to a light source), masks with two or three layers, and masks with inner filter pockets. Novelty or non-protective masks, masks with ventilation valves, or face shields are not authorized as a substitute for masks. Masks must fit properly (i.e., snugly around the nose and chin with no large gaps around the sides of the face).
All individuals on military installations and all individuals performing official duties on behalf of the Department from any location other than the individual's home will follow CDC guidance and practice physical distancing, specifically by staying at least 6 feet (about 2 arm lengths) from other people who are not from an individual's household in both indoor and outdoor spaces. Social distancing should be practiced in combination with other everyday preventive actions to reduce the spread of COVID-19, including by wearing masks, avoiding touching your face with unwashed hands, and frequently washing your hands with soap and water for at least 20 seconds.
THE GOVERNMENT WILL NOT PROVIDE MASKS. IT IS THE RESPONSIBILITY OF THE SITE
VISIT ATTENDEE TO BRING THEIR OWN MASK TO THE SITE VISIT. IF AN ATTENDEE DOES
NOT HAVE A MASK FOR THE SITE VISIT, BASE ACCESS CANNOT NOT BE GRANTED.
2. All participants may be subject to scan or search. Searches/scans may constitute use of guard dogs and/or other detection devices, instruments or security methods/procedures. Security personnel and guard dogs could be present at various locations during the site visit.
3. No participant will be allowed to carry any of these items on the base: cameras or camera phones, knives or weapons, packages or briefcases.
4. No foreign nationals will be permitted to participate in the site visit.
5. All contractors must comply with the completion and submission of the following forms to gain access to the various locations for the site visit:
a. The Department of the Navy Local Population ID Card/Base Access Pass Registration, SECNAV 5512/1 form (Attachment JL-3); and the
b. NSA Panama City Base Access During Covid Lockdown form (Attachment JL-4).
The forms must be completed and returned to Tommy Ivey at tommy.ivey1@navy.mil (with a copy to Courtney Peterson at courtney.j.peterson@navy.mil) by 2:00 PM CDT, Thursday, 6 May 2021.
NOTE: The Government will complete Block 25 on Page 1 of the SECNAV 5512/1 Form.
IMPORTANT ADDITIONAL INFORMATION REGARDING THE SECNAV 5512/1 form:
- SSN must be provided; and
- Blocks 29-31 have to be physically intialed/signed, NOT computer generated.
Additionally, all contractors must bring the completed forms above and the CUSFF/NAVNORTH COVID-19 Screening Questionnaire (Attachment JL-5) to gain access to the location for the site visit. Contractors must use the COVID-19 Unrestricted Travel Tracker (Attachment JL-6) to fill out the screening questionnaire. The COVID-19 Unrestricted Travel Tracker is also located at https://www.mnp.navy.mil/group/don-covid-19-travel-tracker.
Only cleared contractors will be allowed to participate in the site visit.
6. Should special accommodations such as handicapped bus/van access be required to attend the site visit, provide a "Request for Special Accommodations" letter identifying what specific accommodations are needed and how many individuals require these accommodations. This letter must be submitted via email no later than the due date of the SECNAV form. Early submission of requests is highly encouraged. Every effort will be made to accommodate reasonable requests.
7. Each contractor is limited to two (2) participants.
8. Government Transportation will be provided for the NSA Panama City morning tour; however, participants will have to use their own privately owned vehicle (POV) for the NOSC Tallahassee afternoon tour.
9. Participants will not be authorized to exit the bus during site visit unless authorized by Government escort.

No questions will be answered at the site visit. All questions shall be submitted in writing to Contract Specialist, Courtney Peterson, at courtney.j.peterson@navy.mil (with a copy to Cari Fiebach at cari.fiebach@navy.mil) on the Government Preaward Inquiry Form (GPIF), Attachment JL-2. All questions must be submitted prior to the Government Pre-Award Inquiry (GPI) cutoff due date/time of 2:00 PM EDT on Thursday, 13 May 2021. It is the offeror’s responsibility to verify receipt of all questions with the Contract Specialist.
IT SHOULD BE NOTED BY EACH PARTICIPANT, THAT SUBMISSION OF THE REQUIRED
SECURITY FORMS DOES NOT INDICATE ACCESS WILL BE GRANTED. THE SECURITY OFFICE
WILL BE THE DECIDING AUTHORITY IN DETERMINING BASE ACCESS. ALSO, YOU ARE
HEREBY ADVISED BY THIS NOTICE THAT THE LEVEL OF SECURITY AT NSA PANAMA CITY
AND OTHER SITE LOCATIONS MAY CHANGE AT ANY TIME. AS THE LEVEL OF SECURITY
HEIGHTENS, THE AMOUNT OF TIME REQUIRED FOR ENTRANCE TO THE BASE AND OTHER
LOCATIONS ALSO INCREASES. IN ADDITION, THE SITE VISIT MAY BE CANCELED BY THE

ARE RESPONSIBLE FOR ANY COSTS INCURRED TO PARTICIPATE IN THIS SITE VISIT.
