Base Access Procedures

Prepared to Prevail

PURPOSE

Inform contractors and vendors of current base access procedures/updates for AAFB.

Base Access Request/List

To apply for base access Project coordinator (prime contractor) will submit the following documents to the

Contracting Officer (CO) (CAC holder/signature on file with Pass & ID).

Base Access Request (BAR) or Base Access List (BAL) III Log (input name (Last, First, MI), date of birth (yr-mn-dy format) and SSN; leave all other columns blank) Base Access Affidavit (attach documentation/identification if indicated yes on section d, if individual indicates they are born outside of the U.S. then he/she will need to submit proof of U.S. citizenship) Valid photo identification; state issued driver’s license must be complaint w/the REAL ID Act on 15

July 2019 for Guam

After the CO signs the request, it will be submitted to 36 SFS Squadron/Bldg. 23020 or if submitting individual does not have base access still take to Pass & ID/Bldg. 14615 for processing Tracking number will be assigned upon submission; Allow 3-5 business days for processing/approval

Note:

A second form of ID is required when submitting a Guam driver’s license that is not REAL ID compliant (Gold Star top right)

List of acceptable IDs U.S./Foreign Passport Birth Certificate Permanent Resident Card Social Security Card Naturalization Certificate Mayor’s Letter for proof of residency (must be original copy)

Foreign nationals are required to submit a Foreign National Access Request (FNAR) form (valid for 90 days). Permanent Resident card holders are exempt from this requirement The following documentation/identification must be attached to the FNAR

Valid Foreign Passport Valid Work Visa (if applicable) I-94 Form H2B Labor card (if applicable)

FNARs will be submitted to Pass & ID for processing. BARs will be submitted to 36 SFS/Bldg. 23020, provide staff with FNAR tracking number.

Roles/Responsibilities

Project Coordinator Generate Base Access Request and all associated paperwork Generate FNAR when applicable Submit package to CO for endorsement Hand carry package to the respective Base Access Office for processing Notify Pass & ID of employee termination/contract completion

Contracting Officer Designated sponsor for base access Review/endorse BAR and FNAR (verify access days/times)

Visitor Control Center/Bldg. 2403:

Closed

Pass & ID office/Bldg. 14615:

Monday – Friday 0600 – 1800

36 Security Forces Squadron/Bldg. 23020 Monday – Friday 0800 -- 1600

*Paperwork will be accepted 0900 – 1200 and 1300 – 1500 for contracts at the 36 SFS, Pass & ID will accept FNARs from 0800 – 1500, Monday – Friday *Contractor passes are issued from Pass & ID

Both locations are closed on Federal Holidays
