Attachment 5 – Quality Measure Calculations
Enclosure 5 - QUALITY CALCULATIONS
CLS PERFORMANCE WORK STATEMENT
AIMD
The Quality Measure (QM) component of the Performance Measurement Framework is defined as the quantification of Quality achieved by the contractor. It is composed of a measure in the following area:
· Government Surveillance Results (GSR)
Government surveillance is comprised of internal audits, in-process monitoring, and observations conducted by government Quality Assurance personnel. During these quality events deficiencies found may, depending on severity, result in a Corrective Action Request (CAR). Refer to the online Defense Contracts Management Agency Guidebook at http://guidebook.dcma.mil/226/226-1/index.cfm for additional information on CARs. Using CARs as the basis, the GSR measure will be determined using the following rules:
CARMAJ = 0 during the evaluation period then GSR = 1 CARMAJ = 1 during the evaluation period then GSR = 0.75 CARMAJ =2 during the evaluation period then GSR= 0.3 CARMAJ > 3 during the evaluation period then GSR = 0
CARCURE > 0 during evaluation period then GSR = 0 CARCRIT > 0 during evaluation period then GSR = 0 CARREP > 0 during evaluation period then GSR = 0 where
· CARCURE = Number of Level 3 or 4 Corrective Action Requests.
· CARCRIT = Number of Level 2 Corrective Action Requests that have been generated in response to a serious maintenance deficiency that are classified as safety of flight.
· CARMAJ = Number of Level 2 Corrective Action Requests that have been generated in response to a maintenance deficiency that does not impact safety of flight.
· CARREP = Number of Corrective Action Requests that were previously generated and not corrected by contractor, then found in subsequent quality events.
The following formula is used to calculate the Quality Measure:
