8. Preparation Information The Contractor's Safety and Health Plan (S&H) shall describe task and activities of the S&H management at each laboratory and site where the contractor is operational to assure identification, evaluation, and elimination or control of S&H concerns. At a minimum, the plan shall include the following:
a. Requirements specified in NFS 1852.223-73, KNPR 8715.3-1 KSC Safety Procedural Requirements, Volume 1, Safety Procedural Requirements for Civil Servants/NASA Contractors, and NPR 8715.1, NASA Occupational Safety and Health Programs (Chapters 13.2, 13.6, 13.7 and 13.8).
b. Organization charts(s) which illustrates the functional relationships and lines of communication between Safety, the S&H organization, and other organizational elements.
c. Descriptions of the S&H management functions within the organization including the process through which management decisions will be made, including notification to KSC NASA S&H, NE and UB organizations of critical and catastrophic hazards, corrective actions, mishaps, and deviations or waivers to NASA S&H requirements.
d. Description of the responsibility, authority, and accountability of S&H personnel, other contractor organizational elements involved in the S&H effort, and subcontractor S&H personnel. Include the organizational unit responsible for the execution of S&H tasks and the position with the authority to resolve all identified S&H issues.
e. Description of the management controls that will be used to ensure compliance with S&H regulations.
f. Cross-reference of safety program requirements (e.g., OSHA, NASA, local NASA Center, and State) and contractor procedures to avoid duplication of effort.
g. Description of the safety program measurement system, including measurements, metrics, and trend analysis methods. The performance measurement system should describe responsibility for identifying and implementing remedial action, recurrence control, and corrective actions that are necessary to improve performance as indicated by the performance measurement system.
h. A mishap contingency action plan that describes how support and services will continue in the event the contractor has a mishap.
i. Describe the safety analysis methodologies used to identify workplace hazards and mitigations.
j. Describe the electrical safety process.
k. Describe the industrial hygiene process.
l. A Drug and Alcohol-Free Workplace Policy and Plan that complies with NFS 1852.223-74, Drug and Alcohol-Free Workforce.
Update: Revise the S&H Plan when changes occur in items a – l above. Changes to the document shall be identified with revision marks. When an update occurs the revision number of the Plan will be updated.
Format: The S&H Plan shall meet the S&H requirements and address the hazards. After submittal, modification, or update the approved document shall be scanned into PDF format and published electronically into the KSC Business World (Tech Doc).

The Contractor shall utilize KSC Form 6-22, Safety Statistics Record for monthly data submittal and ensure that the report is accurate at the time of the submittal. This includes ensuring that cases that escalate, changes in property damage, and changes in previous year's data are noted on the following month's SSR. Additionally, include an explanation of the changes, and ensure that these cases and changes are updated in the Agency automated data system for mishap reporting.

To provide the Contractor with requirements to report, investigate, and document mishaps, close calls and previously unidentified serious workplace hazards to prevent recurrence of similar incidents.

1. The Contractor shall provide an anomaly report in accordance with KNPR 8715.3-1, KSC Safety Procedural Requirements Volume 1, Safety Procedural Requirements for Civil Servants/NASA Contractors; NPR 8621.1, NASA Procedural Requirements for Mishap and Close Call Reporting, Investigating, and Record keeping (Appendix H); and KSC-PLN-2807, Mishap Preparedness and Contingency Plan, Chapter 4.
2. The anomaly report shall be prepared and distributed to organizations identified in Block J no later than 4 hours after the incident occurs and shall include, but not limited to:
a. Date, time, location and nature of anomaly
b. System or Systems involved
c. Number, and if possible, nature of any injuries
d. Probable cause of concurrence and events leading up to anomaly
e. Actions taken to eliminate or minimize any hazards or further damage
f. Planned or corrective actions to prevent recurrence, if known
g. Persons notified
h. Photographs, if available
3. Anomalies that meet the criteria of NASA Mishap for Type A through Type D as defined in KNPR 8715.3-1, shall be upgraded to Mishaps and electronically entered into the Agency automated data system database for mishap reporting within 24 hours of the anomaly or by the end of the next regular duty day if the accident occurs after 15:00 hours on the last work day before a holiday or weekend or anytime on weekends or holidays. In addition, all anomalies with the potential to have been a Type A through Type D Mishap shall also be entered. Data entered shall contain as much detail as available and when appropriate shall include details such as root cause analysis, measures to prevent recurrence, photographs, etc.

2. Environmental workforce structure, management hierarchy, and interfaces throughout the contract to ensure communication of sustainability requirements, environmental requirements, policies, and procedures.
3. Procedures that will be implemented to ensure environmental compliance and perform internal surveillance.

The Contractor shall prepare, maintain, update, implement, and comply with site-specific SPCC plans for the oil storage locations, transfer activities, and assigned equipment. Site-specific SPCC plans must comply with Title 40 Code of Federal Regulations Part 112 regulations and the KSC-PLN-1919, KSC Facility-Wide SPCC Plan.
During semi-annual update calls, in accordance with KSC-PLN-1920, Appendix B: KSC Site-Specific SPCC Plans, the Contractor shall:
1. Review, validate, and update existing site-specific SPCC plans.
2. Delete site-specific SPCC plans for removed oil storage activities and locations.
3. Develop site-specific SPCC plans for new oil storage activities and locations.
4. Prepare an amendment log to document and summarize all site-specific SPCC plan changes, additions, and deletions.
5. Arrange for a licensed professional engineer (PE) to review and certify all non-administrative site-specific SPCC plan changes, additions, and deletions.
6. Submit SPCC plan changes, amendment logs, and PE certification sheets in the format/method and by the due date described in the data call.

Quality Management Plan (QMP)
The Contractor shall provide the initial submittal of the Quality Management Plan (QMP) within 60 business days after contract award for Government review and approval.
The Contractor shall review the plan annually and revise as required thereafter, to reflect the process of the Quality Management System (QMS) and submit for Government concurrence.
The Contractor shall revise the QMS and QMP as required when organizational changes occur that change the organization executing the quality requirements, change the quality management functions (e.g., the process through which management decisions are made, or change the organizational responsibility, authority or accountability) and submit to the Government within 30 days of the revision. The Kennedy Space Center quality organization may perform assessments to verify compliance.

The report shall be in the form of a briefing that addresses highlights of the past 6-months of contractor activity. The purpose of this report is to provide NASA management and the Contractor a basis for discussion on the performance of contract activities. The LASSO II Contractor shall also address any issues of concern and operational success.

Advance Notification of Workforce Reductions Report

The Contractor shall notify the Government in writing at least 15 calendar days in advance of notification to employees of any planned reductions of permanent, full time and part time employees that exceed 10% of the total contract workforce or 25 employees, whichever is less.

Advance Notification of Workforce Reductions Report

To provide the Government advanced notification of workforce reductions.

Pursuant to the Workforce Adjustment and Retraining Notification (WARN) Act, the Contractor shall provide in writing to the Government the information required by the WARN Act, section 639.7(c) and a listing of all organizations to be notified including those required by section 639.6(a), (c) and (d) at least five (5) days prior to issuance of the notification.
The notification shall include the reason for the reduction in workforce, the number of employees impacted, their labor classification, their physical location and their location within contract organizational structure.
The notification shall also identify the expected date of the first separation and the anticipated schedule for making separations, as well as the name and telephone number of a contact for further information.

Definition of Terms
a) On-site – Number of Contractor personnel occupying physical space at KSC. KSC includes Cape Canaveral Air Force Station (CCAFS). It includes those personnel temporarily absent from assigned duty stations (e.g., on leave without pay, annual/sick leave, TDY).
b) Off-site - Number of Contractor personnel physically located outside of KSC property (as defined above).
1. Dispossessed – Number of Contractor personnel who would normally occupy physical space at KSC (as defined above) but who are located outside of KSC property (as defined above) due to non-availability of space.
2. Others Off-site – Number of Contractor personnel who are intentionally, physically located outside of KSC property (as defined above).
c) Subcontractors – Number of subcontractor personnel occupying physical space at KSC (as defined above). Include the subcontractor name and a brief description/title of the effort.

Quarterly Summary of 3rd Step Grievances and Arbitrations Report

The report shall include:
a. Number of open 3rd step labor grievances at the beginning of the current quarter.
b. Number of 3rd step labor grievances resolved during the quarter (with out arbitration).
c. Number of open 3rd step labor grievances at the end of the quarter.
The report shall include a summary paragraph of the most common grievances experienced during the quarter.
The report shall also contain a separate paragraph reporting the number of arbitrations in which the Contractor was involved during the quarter, a brief description of each and summary of the decision.

Training Plan

The Contractor shall review and submit the Training Plan for Government approval annually by 1 October. As required, interim modifications shall be incorporated and submitted to the Government for approval.

Training Plan

DR 1.3-5, Training Records, Quality Assurance Plan
The Contractor shall provide a trained, certified and licensed, as necessary, workforce to satisfy contract requirements. The Contractor shall ensure the workforce is knowledgeable of the applicable laws, regulations, and government directives.
The Training Plan shall include technical training; quality, safety, health, environmental and operational area access training;
professional development; certifications; and licenses. Each professional and technical member of the Contractor’s staff shall possess an academic degree in the discipline or an equivalent combination of education and experience suitable to the responsibilities of their position. The Training Plan shall consist of the following:
a. KSC-Provided Training Needs Assessment The training plan shall contain a training needs assessment which will provide a forecast of courses to be provided to LASSO II by the NASA Technical Training Services contractor (KISS). These classes are standardized for use by all KSC organizations (NASA and contractors) and generally consist of classes such as area access, safety, environmental and hazardous material handling. The assessment shall provide a one-year projection, in monthly increments, of the classes and the estimated number of personnel requiring training for each class. The Contractor shall communicate with the provider to identify real-time training requirements.
The Contractor shall coordinate with the provider to ensure complete understanding of the training catalog (available courses), course duration and length, and contents to ensure satisfaction of LASSO II requirements.
b. LASSO II-Provided Training Approach The second part of the training plan shall describe the LASSO II training approach. The Contractor shall list and describe the skills and certifications and licenses, including any identified critical skills, required to support LASSO II activities through the contract performance period. The Contractor shall describe how these requirements meet staffing levels, personnel utilization and skill mix needs.

Export Control Plan

The Contractor shall provide the initial submittal within 30 days after contract start. The Contractor shall provide the final submittal within 120 days after contract start for Government approval. The Export Control Plan requires approval from the Center Export Administrator (CEA). The plan shall be reviewed at least annually thereafter and updated as required.
The Contractor shall submit type "D" electronic copies that are compatible with Microsoft Office software or Adobe (*.pdf). The Contractor shall also publish this DR electronically in a Government-accessible location.

To document the Contractor requirements to ensure that exports and transfers of commodities, technical data, or software to foreign persons and foreign destinations are carried out in accordance with US export control laws and regulations, and Administration and NASA policy
PWS 1.5.2
The Contractor shall develop, implement and maintain an Export Control Plan. The plan shall consist of three parts:
1. The plan shall describe all export control activities related to the performance of LASSO II requirements per NPR 2190.1, NASA Export Control Program. The Contractor shall prepare and submit an Export Control Program (ECP) plan to describe the Contractor's planned approach for accomplishing contract functions while strictly adhering to the export laws, regulations and directives. Topics should include as a minimum the functions addressed in NASA Advisory Implementing Instruction (NAII) 2190.1, NASA Export Control Program Operations Manual.
2. The plan shall also capture the following workload indicators:
a. Number of Export Classifications performed (drawings, plans, imagery, presentations, etc.)
b. Number of foreign shipments and export authorizations utilized. (Any foreign shipments or export authorizations utilized shall be reported to the NASA Export Control Office on a monthly basis.)
c. Number of close calls and violations.
3. The plan shall include Export Control Facility Plans (ECFP) for buildings and areas often visited by external entities. The ECFP shall identify the necessary steps and precautions to be implemented by the Contractor to ensure the protection of NASA Export Controlled hardware, technical data and technology.

Laboratory Equipment Maintenance Plan

An individualized Maintenance Plan will be generated for each Lab listed in Appendix 3. Each Maintenance Plan will be comprised of three sections: Section 1 describes the Contractor's approach for maintenance of laboratory equipment, systems and assets.
Section 2 is an annual listing of laboratory equipment, systems and assets for maintenance. Section 3 contains a quarterly report of maintenance performance to plan.
The Contractor shall provide initial submittal of Sections 1, 2, and 3 for its overall Maintenance Plan methodology and any specifics associated with the supported laboratories' equipment, systems and assets. The Contractor shall provide the initial submittal within 90 days after contract start date and then quarterly thereafter for Government review and approval.

DR 2.2-2 Preventive Maintenance Instructions
The Maintenance Plan is comprised of three sections: Section 1 describes the Contractor's approach for maintenance of laboratory equipment, systems and assets. Section 2 is a listing of laboratory equipment, systems and assets for maintenance. Section 3 contains a report of maintenance performance and cost trends, as described below:
Section 1:
The Contractor shall develop, implement and maintain a Government-approved laboratory equipment maintenance plan for equipment, systems, and assets consistent with Government requirements and original equipment manufacturer specifications.
The maintenance approach shall include a methodology to:
1. Maintain laboratory equipment, systems and assets in the most cost-effective manner to minimize risk to processes and products, protect the safety and health of personnel and the environment, and protect and preserve capabilities and capital investments
2. Identify, schedule, plan, manage and track maintenance activities
3. Identify management techniques that optimize maintenance activities with respect to risk management and cost
4. Improve reliability and maintainability
5. Trend and analyze data to make recommendations for proposed improvement upgrades or replacements
6. Determine costs associated with repairs and consumables (e.g., materials, tools, equipment, or general supplies)
7. Ensure trained personnel are performing maintenance activities
Section 2:
The Contractor shall provide an updated listing of laboratory systems, equipment and assets that LASSO II is responsible for maintaining. The Contractor shall coordinate with the Government to identify maintenance requirements for the NASA-managed ES&R laboratory systems, equipment and assets.
Section 3:
The Contractor shall provide a quarterly report of maintenance performance and cost trends by laboratory. The report shall include the following:
1. Maintenance budget planned for the current fiscal year
2. Annual trend and assessment of actual versus planned preventive maintenance costs over contract period (including labor and other direct costs)
3. Annual trend and assessment of actual versus planned corrective maintenance costs over contract period (including labor and other direct costs)
4. Annual trend summary of monthly total preventive and corrective maintenance hours expended
5. Annual trend summary of monthly ratio of performed versus scheduled preventive maintenance tasks
6. Current system, equipment and assets condition assessment data (i.e., correlates to obsolescence equipment calls)
7. Annual assessment of unscheduled system and equipment downtime, along with resulting corrective action(s) implemented to preclude re-occurrence

Instructions for Completing Data Requirement Description
General - The Data Requirement Description (DRD) will be prepared to describe the content and provide preparation information for data required in support of NASA programs.
1. Title - Enter the title or type of document required. The first word of the title should be a principal noun which best establishes the basic concept of the data. Subsequent words should be appropriate modifiers.
Examples: Plan, Project Development (SIVB); Specification, Test (GSE); Report, Quarterly Progress; Proposal, Engineering Change (ECP)
b. Stipulation of specific forms when multiple forms are authorized on the DRD.
2. Number - Enter the appropriate number assigned to the DRD. This number will identify the appropriate data category.
3. Use - Enter a synopsis of the use of the document, stating reason for the requirement.
4. Date - Enter date of preparation.
5. Organization - Identify the installation preparing the DRD.
6. References - List applicable documents by number, (NASA Management Manual, Mil Specifications, Federal Standards, NASA Procurement
Regulation, etc.) to which the preparing office (e.g., NASA installation, contractors, etc.) may refer for additional information concerning the data requirement.
7. Interrelationship - Enter all affected approved DRDs within the scope of the program when the DRD under preparation creates a significant impact or interface relationship with existing DRDs. Include a brief narrative of the impact or relationship created and a statement that the new DRD does not cause a conflict with other DRDs.
8. Preparation Information - Provide ample information for preparation of the data required by the data requirements description; include all necessary details of preparation to satisfy the originator's formal requirements.
