LABORATORY SUPPORT SERVICES AND OPERATIONS II (LASSO) SOLICITATION

John F. Kennedy Space Center, Florida

This questionnaire is to be submitted for evaluation of a company that has intentions to compete for the LASSO II contract as a prime or subcontractor. The following terms, “customer reference” and “evaluator,” are used to refer to the person performing the evaluation. For the LASSO II Request for Proposal (RFP), NASA requires the company to provide this questionnaire to customer references for which it has had an active or completed contract within the past three (3) years. Please complete this questionnaire and return to NASA per the following instructions. Your assistance in this effort is greatly appreciated.

The scope of the LASSO II contract provides support services including laboratory equipment maintenance; operational laboratory services for Component Cleaning and Refurbishment, Chemical Sampling and Analysis, Calibration and Standards, and Nondestructive Evaluation; and research and technology (R&T) scientific support and engineering support within various laboratories located at John F. Kennedy Space Center (KSC). The LASSO II laboratories support the KSC Institution, NASA’s programs and projects, Cape Canaveral Space Force Station, and a growing base of spaceport customers.

Instructions for completing the Past Performance questionnaire and a sample cover letter to send to your customer reference for performance assessment are included in the following pages. Please request that the customer reference complete the attached questionnaire and email a PDF file, to Michael Wheeler, Contracting Officer, at ksc-lassoII-team@mail.nasa.gov. The completed questionnaire will become a part of the official source selection records. To be appropriately considered, the Past Performance Questionnaire shall be returned to the Contracting Officer in its entirety no later than: January 19, 2022. The National Aeronautics and Space Administration (NASA) Kennedy Space Center (KSC) is in the process of soliciting offers from companies for the Laboratory Support Services and Operations (LASSO) II contract. ______________________ has provided your name and organization as a reference regarding our record of past performance under Contract Number: _______________________.

Your completed questionnaire will become a part of the official source selection records. To be appropriately considered, the Past Performance Questionnaire shall be returned to the Contracting Officer in its entirety no later than 3:00 p.m. KSC local time, on January 19, 2022.

QUESTIONNAIRE INSTRUCTIONS

Sections I - Company and Contract Information: To be completed by the Offeror.

Section II - Evaluator Information: To be completed by the evaluator.

Sections III: Performance Assessment: To be completed by the evaluator. Provide your assessment of the company’s performance by marking the appropriate rating for the contract for which you are a customer reference. It is very important to keep in mind that only performance on active or completed contracts in the last three years is relevant.

For each item in the Performance Assessment, respond by placing an “x” in the appropriate block with the rating that best describes the company’s performance for the referenced contract or order.

The LASSO II Contract Source Evaluation Board may contact you regarding the information you provide.

The rating scale is defined as:

Exceptional (E)
Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.
Very Good (VG)
Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the Contractor was effective.
Satisfactory (S)
Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the Contractor appear or were satisfactory.
Marginal (M)
Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory (U)
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the Contractor’s corrective actions appear or were ineffective.

No information or does not apply to this contract
