Past Performance Information Sheet
GENERAL INSTRUCTIONS:
1. Offeror must answer all questions.
2. This form must be completed by the Offeror.
3. Offeror must complete a separate Past Performance Information (PPI) Sheet for each of the Corporate Experience contract included under the Corporate Experience Factor.
4. The Offeror must provide a copy of the completed PPI with the proposal submission.

The following items a through 2 should be addressed in narrative form. Item f must be addressed in narrative form:
a. Quality of Service: The offeror should describe past compliance with contract requirements, accuracy of reports, appropriateness of personnel, technical excellence.
b. Timeliness of Performance: The offeror should detail how it met milestones, provided reliable delivery and continuity of services, was responsive to technical direction, and completed work on time.
c. Customer Satisfaction: The offeror should describe satisfaction of customers with the contractor’s service and product.
d. Business Relations/Integrity/ Effective Management: The offeror should detail its business relations with the customer on previous contracts with emphasis on its effectiveness to manage the contract requirements, subcontracts, and personnel while completing work on time.
e. Ability to Manage Cost/Control Cost: The offeror should describe past efforts to control costs under previous contracts.
f. Environmental Violations/Enforcement Actions: The offeror must detail all past environmental violations and/or enforcement actions submitted against the offeror as well as those submitted against a customer as a result of the offeror’s performance. (For example, a Government agency received a violation based on offeror’s performance). The outcome/remedy of any violations and/or enforcement actions must be included.
