692M15-19-R-00005 Amendment 0004 Page 3 of 3 Amendment 0004 is issued to provide answers to all submitted questions. The following documents are hereby included under Amendment 0004:

In addition:
1. The Project Geologist/Hydrogeologist is a full-time, on-site position which has been clarified in the conformed version of the SIR that is released with this Amendment.
2. Standard Form SF-30 is provided for Amendment 0003, as Amendment 0003 was inadvertently posted without same. Offerors must sign the SF-30 for each of the 4 SIR Amendments and submit with their Phase IIII proposals.
3. SIR Section L.4.8 ANNEX #1 is hereby amended and updated to include the following under the list of required submissions:
(7) Completed NDAA Representations Letter in Attachment L-0004, NDAA Representations Letter. Failure to provide these representations will preclude an Offeror from being eligible for award.
4. SIR Section L-004- NDAA Representations Letter is hereby attached.
5. SIR Section L.5.7.1.1 Responsibility Determination is hereby added in Section L, under Section L.5.7.1.
6. Standard Form SF-24, Bid Bond, Standard Form SF-25A, Payment Bond and Standard Form SF-25 Performance Bond are attached.
7. A corrected SIR Section L-003 Past Performance Information Sheet is attached.
8. The following SIR Section L- Evaluation Factor requirements are due by January 4, 2021 by 5 P.M., EST:
· L.5.4 Phase III, Volume II, Factor III- Sample Task
· L.5.5 Phase III, Volume II, Factor IV- Key Personnel
· L.5.6 Phase III, Volume II, Factor V- Past Performance
· L.5.7 Phase III, Volume III Price
9. Conformed versions of SIR 692M15-19-R-00005, Conformed SOW, and Conformed B-001 Pricing Schedule are attached.
10. All correspondence sent to the FAA from this point forward must be sent to Lori.McLaughlin@faa.gov, with a copy to Daniel.Farrell@faa.gov. There is no longer a need to copy Kimberly.A.Knight@faa.gov.
