Instructions for Proposal Cost Template in Response to Request for Proposal HSBP1014R0058
1. General Approach: The Proposal Cost Template Excel workbook consists of 28 tabs or worksheets. Offeror data or input is required in 19 of the tabs and is calculated and linked to the remaining 9 tabs.
2. Offeror data: Offeror data or input will include the information necessary to complete the Proposal Cost Template. Data will include Labor Categories and related Labor Rates, Units for Activity, Number of Activities, Travel Time per Activity, Materials and related Unit Rates, Equipment and related Unit Rates, and Other Direct Costs, as appropriate. In order to facilitate government analysis, offerors shall not lock cells containing offeror data.
3. Government Links and Formulas: The government uses links and formulas to calculate and combine amounts, as appropriate. The links and formulas shall not be altered.
4. Definitions: Key definitions are provided below.

| Units of Activity |
| “Units of Activity” varies depending upon the location in the spreadsheet. In the labor category, “Units of Activity” refers to labor hours to accomplish the work activity described in the Sample Work Plan; in the materials category, it refers to the quantity of material needed to accomplish the work activity, and in the equipment category it refers to the number of pieces of equipment need to accomplish the work activity. |

| “Number of Activities” refers to the number of times the activity is to be accomplished as defined in the Sample Work Plan. |
| Travel Time per Activity |
| “Travel Time per Activity” is the travel hours as define in the Sample Work Plan |
5. Spreadsheet Tab Guide: Details concerning data entry are provided below.

4. No other offeror data input is required on this tab.
5. Cell D54 contained the government provided amount $2,250,000.00 for “(M) CLIN 9 - Improvement Activities”. The offeror shall not change this amount. The amount includes all overheads and profit. The amount is not escalated but remains the same in each of the option periods.

| Wage Labor Rates |
| The purpose of this tab is to allow the offeror to display the base pay and components of pay that lead to the “wage labor rate(s)” used in other parts of the proposal cost template. Also, the offeror may choose the alter some of the suggested headings for the components of the “buildup” of the wage labor rates. However, each wage labor rate used in the template must be detailed in this table. The offeror may “insert” additional rows in any tables as appropriate. |
The Hourly Rate for Labor Category – Construction and the Loaded Hourly Rate for Labor Category – Services in the “Wage-Labor Rates” tab should match up to the detailed F&G, R&B, D&G, L&E, and V&D requirements for sectors DRT, LRT, & RGV by labor category. While this information is used to support and justify the labor rates entered in other tabs in the Cost Template, there shall be no data links from the “Wage-Labor Rates” tab to any other tab of the Cost Template.
