About this file

This is a Reference Questionnaire Form issued by Collier County's Procurement Services Division in Florida for evaluating the qualifications and past performance of firms and key personnel competing for professional services contracts. The form is designed to be completed by previous clients of proposing firms and must be submitted with proposal responses. Collier County utilizes this reference collection process as part of its firm selection methodology for projects such as Design Services for Veterans Memorial Boulevard Phase II. Evaluators are required to complete one form per required reference, providing comprehensive feedback on contractor performance across multiple dimensions.

The questionnaire requests evaluators to rate ten specific performance criteria on a scale of 1 to 10, with 10 representing high satisfaction and likelihood of future engagement and 1 representing dissatisfaction and unwillingness to rehire. The rated criteria include project cost management and change order minimization, schedule adherence, work quality, consultative advice quality, personnel management and professionalism, project administration and documentation, communication clarity, risk management capability, contract compliance, and overall customer satisfaction. Evaluators must also provide contextual project information including project description, completion date, project budget, and project duration. Any criteria left blank due to insufficient knowledge will receive a score of zero. The form requires completion of all mandatory fields and submission with the proposal package to be considered in the evaluation process.
