About this file
This document is Form 10 - Certification of Drug-Free Workplace Program for MyFloridaNetwork-3, issued by the State of Florida's Department of Management Services (DMS) under Invitation to Negotiate (ITN) No. DMS-24/25-260. The form outlines the requirements for businesses to establish and maintain a drug-free workplace program in order to receive preference in the state procurement process when bids are otherwise equal in price, quality, and service.
The document details six specific requirements for implementing a drug-free workplace program, including publishing employee statements about controlled substance prohibitions, informing employees about drug abuse dangers and assistance programs, providing written statements to employees, notifying employers of drug-related convictions, imposing sanctions or rehabilitation requirements for convicted employees, and making a good faith effort to maintain a drug-free workplace. The form provides a certification section for a respondent's authorized representative to sign, affirming compliance with these requirements, with a warning that false statements are punishable by law.
