HILL AIR FORCE BASE, UTAH
ASBESTOS SURVEY FOR BUILDING 2013
RENOVATE FACILITY
During February 2009, Mr. Al Lewis with 775 th
CES requested an asbestos survey of building
2013, to provide information for a project to renovate the facility. The scope of this project is as follows:
Work Location: Building 2013
Work Order: # 49980
Statement of Work:
Renovate the facility.
The information provided for this survey is based on samples collected by Nickolas King (Utah
Certified Asbestos Inspector ASB-#2257). Samples for this survey were collected on 25
February 2009. Dixon Information Inc., utilizing EPA Method 600R-93-116, analyzed all samples. EPA Method 600R-93-116 is similar to NIOSH Manual of Analytical Methods 9002.
Civil Engineering personnel also reviewed all available asbestos survey databases of suspect asbestos containing building materials (ACM) that could potentially be encountered in this area.
The information gathered from all current and previous surveys is shown below by homogenous area.
Drywall/joint compound wall system (6000’ sq) was sampled from the southwest corner of room
8, from the northeast corner of room 17, from the southwest corner of the gear box repair area, from the south side of the small clean room, from the southwest corner of the hall, from the northeast corner of the men’s restroom, from the northeast corner of the large clean room, and from the south side of the small clean room. These samples were analyzed as a non-asbestos containing material (GM091201, GM091202, GM091203, GM091204, GM091205, GM091206, GM091207, and GM091208).
Thermal system hard tank insulation, (500’ sq) was sampled from the east side of the tank in mechanical room #2, from the east side of the tank in mechanical room #2, and from the west side of the tank in mechanical room #2. These samples were analyzed as a non-asbestos containing material (GM091209, GM091210, and GM091211).
Thermal system pipe insulation, white/yellow (1000’ ln) was sampled from the center of the east side of the attic, from the center of the west side of the attic, from the center of the south side of the attic, from the center of mechanical room #1, and from the center of the south side of mechanical room #2. These samples were analyzed as a non-asbestos containing material
(GM091212, GM091213, GM091214, GM091215, and GM091216).
Thermal system pipe fitting insulation, white/yellow (150 ea) was sampled from the center of the attic, from the northwest corner of the attic, from the southeast corner of the attic, from the center

1425 hours of mechanical room #1, and from the center of the east side in mechanical room #2. These samples were analyzed as a non-asbestos containing material (GM091217, GM091218, GM091219, GM091220, and GM091221).
Batt insulation, foil/yellow (13,000’ sq) was sampled from the southeast corner of the attic, from the northwest corner of the attic, and from the center of the south side in the attic. These samples were analyzed as a non-asbestos containing material (GM09122, GM091223, and GM091224).
Duct insulation, foil/yellow (3000’ sq) was sampled from the center of the north side in the attic, from the center of the west side of the attic, from the center of the east side of the attic, from the center of mechanical room #1, and from the center of the north side in mechanical room #2.
These samples were analyzed as a non-asbestos containing material (GM091225, GM091226, GM091227, GM091228, and GM091229).
2’ x 4’ white ceiling tile, pockmarks/pinholes (7500’ sq) was sampled from the southeast corner of the hall, from the northeast corner of the hall, from the southwest corner of the break room, from the center of the gear box repair room, and from the northeast corner of the MIC. These samples were analyzed as a non-asbestos containing material (GM091230, GM091231, GM091232, GM091233, and GM091234).
2’ x 4’ white ceiling tile, wormy grooves/pinholes (7500’ sq) was sampled from the southeast corner of the hall, from the northeast corner of the hall, from the northeast corner of the MIC, from the southwest corner of the break room, and from the center of the gear box repair room.
These samples were analyzed as a non-asbestos containing material (GM091235, GM091236, GM091237, GM091239, and GM091240).
12” x 12” floor tile, gray/yellow adhesive (3000’ sq) was sampled from the southeast corner of room 17, from the southeast corner of room 15, from the northwest corner of room 19, from the northeast corner of the locker room, and from the southwest corner of the MIC. These samples were analyzed as a non-asbestos containing material (GM091241, GM091242, GM091243, GM091244, and GM091245).
12” x 12” floor tile, tan/yellow adhesive (1500’ sq) was sampled from the southwest corner of room 8, from the southeast corner of the south entrance, and from the northeast corner of the hall. These samples were analyzed as a non-asbestos containing material (GM091246, GM091247, and GM091248).
3’ x 3’ floor tile, gray/black adhesive (1500’ sq) was sampled from the southeast corner of the large clean room, from the southwest corner of the large clean room, and from the southeast corner of the small clean room. These samples were analyzed as a non-asbestos containing material (GM091249, GM091250, and GM091251).

Sheet vinyl flooring, tan/yellow adhesive (500’ sq) was sampled from the northwest corner of the women’s restroom, from the south side of the women’s restroom, and from the east side of the women’s restroom. These samples were analyzed as a non-asbestos containing material
(GM091252, GM091253, and GM091254).
Roofing shingles, brown/black felt paper (13000’ sq) was sampled from the center of the south side of the roof, from the southeast corner of the roof, and from the southwest corner of the roof.
These samples were analyzed as a non-asbestos containing material (GM091255 GM091256, and GM091257).
All Hard Thermal System Insulation not mentioned in this survey; encountered above ceilings, behind walls or in wall chases, in attic spaces, and in mechanical rooms is assumed to be asbestos. This includes all insulation on pipe runs, pipe saddles, tanks and elbows.
Do not move, cut, or disturb this material in any way.
All 9” floor tile and associated black mastic adhesive is assumed to be an asbestos containing material. Do not disturb this material.
Remember that the locations of these building materials (homogenous areas) indicate where these materials were found, but do not necessarily mean that these are the only locations where these materials may be encountered during the project. When any new suspect ACM is encountered, stop work and contact either CEOHA or SGPB to identify any new suspect building materials.
DO NOT DISTURB ANY BUILDING MATERIALS THAT ARE SUSPECTED ACM.
CONTACT TROYE DAVIS AT 586-7094 TO ARRANGE REMOVAL OF THESE
MATERIALS IF THERE IS ANY POTENTIAL FOR THEM TO BE DISTURBED
DURING THIS PROJECT.
The project manager must post a copy of this survey at the project before this project begins, and must remain posted until the project is completed. Please contact Troye L.
Davis at 586-7094 with any questions regarding this survey letter.
