1 HAMS 9 Mar 12: Response to Question #12 – Clearly states Subfactors

1.1 and 1.2 are NOT to be included in the MFAM. The Transition Plan, Work Schedules, Staffing Plan, Contractor-Furnished Equipment and

Supplies are to be part of the offerors proposals in addition to the

MFAM. Q: Please clarify the response, does in addition to the MFAM mean the items listed above from Subfactor 1.3 are or are not included as part of the MFAM along with the elements described in PWS Para

2.1.18? In other words, is it correct that Subfactor 1.3 Parts 1) thru 6) are the headings for the MFAM contents?

The requirements listed in Subfactor 1.3 are transition plan, work schedules, staffing plan, contractor furnished equipment and contractor furnished supplies, as well as the MFAM. These are not part of the MFAM. The requirements for the MFAM are listed in the PWS, 2.1.18.

2 PWS 1.3.27 Pre-Cleaning is defined as the daily complete and thorough cleaning performed prior to the start of day or use of the area. Q:

Does pre-cleaning only apply to RS #2 and RS #6 rooms/areas?

Yes

3 PWS 2.1.6.3 States that Critical Care rooms/areas listed in RS items 1, 2, 5, and 6 are the only rooms/areas that do not require a written schedule. Subfactor 1.3 Part 3) Work Schedules – Requires sample work schedules for Keesler Operating Rooms/Surgical Suites which are considered Critical Care rooms/areas.

Q: Are work schedules required for Critical Care rooms/areas which includes the Operating Rooms/Surgical Suites?

For the sample task, offerors should provide general work schedules for operating rooms/surgical suites as required in the IMFE. After task order award, written work schedules for these areas will not be required.

4 PWS 2.1.7 Dedicated Personnel – States that sufficient dedicated personnel shall be permanently assigned to meet the case-driven response times… supplemental personnel shall be trained and assigned to fill in during breaks, lunches, or any other absence of the permanently assigned personnel…

Q: Please clarify permanently assigned, does this mean no employee change once an employee has been assigned to the case-driven area

(OR/L&D), or that employee must be assigned to case-driven area only?

Q: Is it correct that supplemental personnel assigned to fill in during breaks, lunches, or any other absence only applies for employee positions in the Operating Rooms and Labor & Delivery Area?

Q1: Individual employees can change as along as the requirements are satisified by a qualified individual.

Q2: Yes

5 PWS 2.1.7 NOTE: Personnel shall be dedicated 24 hours per day in labor and delivery areas.

Q: Following the end-of-day tasks is it correct that housekeeping personnel need only be available to respond to L&D within required time frame?

Yes

6 IMFE for Keesler RS #2 and RS #6 both state the same personnel shall not cover both surgery and labor/delivery "case driven".

Q: Does this mean that different personnel must be assigned for necessary after hour response to RS #2 and RS #6 during the second and third shift?

No. Personnel must meet response time to both areas (if needed) at the same time.

7 PWS 2.1.10.6 States mixing of chemicals shall be accomplished in areas having permanent eye wash stations only.

PWS 2.3.6 Government Furnished items include permanently installed eye wash facilities, where required.

Q: How many areas within each location currently have permanent eye wash stations installed where employees can mix chemicals as needed during their assigned shift?

Q: Is contractor responsible for additional installation of permanent eye wash stations where deemed necessary?

Specific data on location of eye wash stations is not available. Each MFT is different and will have different locations. The Contractor is not responsible for installation of permanent eye wash stations. The government will provide eye wash stations as required.

8 PWS 2.1.17.2 Pre-Move in Cleaning is required when a construction contract has been completed, the construction contractor has completed the cleaning required by the construction contract and the area is ready to move into. Q:

What level of cleaning is required when completing construction contracts? Example - If construction contractor is required to only broom sweep, then Pre-Move in Cleaning would include (but not limited too); washing dust from all walls, furniture, and horizontal surfaces, stripping, sealing, and finishing all floor area, etc.

A Pre move in cleaning shall bring the area up to contract compliant level of asepsis and cleanness.

9 PWS 2.1.18.3 Explains the components required to be addressed within the contractor’s Quality Control Program and the final component listed states…and shall also include bacteriological monitoring when necessary.

Q: What type of bacteriological monitoring process may be necessary to be performed, and who will determine when and where it is necessary?

A contractor may be instructed to do certain monitoring if conditions warrant as determined by the contracting officer with consultation from a government infection control expert. Specific instructions will be provided at that time based on the specifics of the situation.

10 PWS 2.1.23.3 States contractor employees shall be immunized annually with the seasonal influenza vaccine and any other vaccine recommended…

Q: Do contractor employees have the option to decline annual influenza vaccine or any other recommended vaccine, and if declined for whatever reason what is the ramification?

The contractor should provide a written declination to the contracting officer, signed by the employee, stating reason why they cannot be inoculated. The employee might not be allowed to work in certain areas of the facility.

11 PWS 2.1.27 States the contractor shall move all furniture and equipment to allow for cleaning underneath and behind…

Q: Does this include; desks, hooked-up computers, monitors, and printers, full filing cabinets, refrigerators, exam room tables, medicine cabinets, and personal belongings?

Yes, unless exempted by the PWS and IMFE. “Total Clean” is the requirement.

12 PWS 2.1.36.1 Criminal History Check will be conducted on all prime and sub-contractor employees requiring base access…Within 10 business days Security Forces will notify the contractor…

PWS 2.1.37.1 States all contractor employees shall have a favorable

National Agency Check with Inquiries application initiated by company’s POC, and submitted to OPM…

The two above PWS paragraphs seem to reflect that the criminal history check is performed by security forces, and that the contractor submits applications for NACI check to be performed.

PWS 2.1.37.5 States that the contractor shall be responsible for any cost associated with security clearances (if they hire anyone who does not possess a clearance).

Q: What are the costs associated with the criminal background check, and NACI checks performed on contractor employees?

Q: What is required for contractor employees to be issued a Common

Access Card?

Q1: Costs shall be determined by the contractor in accordance with their cost account standards.

Q2: To be issued a CAC, a contract employee must have a favorable NACI.

13 PWS 2.3 Government/Contractor Furnished

Services/Information/Property/Materials.

Q: Is contractor permitted to have private phone installed for use?

Q: Is contractor permitted to furnish their own computer, monitor, and printer for company use?

Q1: No

Q2: This will be up to the individual MTF Facility Manager and its Security

Office. However, contractor furnished computers/monitors/printers will not be connected to the AF Local Area Network. Please refer to PWS 2.3.10

14 PWS 2.3.8.2 States government will furnish tele-communications pagers and/or two-way radios,…

Q: Does this apply to all 51 contract locations?

The contractor will be provided supplies, materials and equipment in accordance with the PWS para 2.3.8.

15 PWS 3.2 Quality Control Program – States the contractor shall develop quality control procedures addressing the areas identified in Technical

Exhibit D, Performance Requirements Summary.

Q: Where is Technical Exhibit D within the solicitation material?

There is no Technical Exhibit D in the Section L or Section M. PWS para 3.2 last sentence will be removed from the PWS at time of contract award.Technical Exhibit D does not exist. The last sentence of paragraph 3.2 should be discounted. Paragraph 3.2 should read as: 3.2 Quality Control

Program. The Contractor shall maintain a quality control program that ensures

“green” environmental services are performed in accordance with commonly accepted commercial practices, voluntary consensus standards, and in compliance with federal, state, installation, and MTF policies, procedures, and regulations as identified in this PWS. The Contractor shall provide procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur.

16 Section M – Evaluation Factors for Award, Para. 1.(b) …Evaluation and award will be based on lowest price.

Q: Is there any provision to limit the number of awards one contractor can receive out of the 51 contracts available, or will initial lowest price

(after being found technically acceptable) be the only deciding factor to receive the awards?

Please see Solicitation clause 52.516-19 Order Limitations. Award will be made in accordance with Section L and Section M of the solicitation.

17 CBA Addendum A for Keesler indicates two different wage classifications; Housekeeper and Group Leader, but the Keesler Wage

Worksheet only has a box for Housekeeper.

Q: In what box on the Wage Worksheet (or other worksheet) are contractors to include the additional cost of all Group Leader hours for

Keesler and all other facilities that have two wage classifications?

The contractor will determine the appropriate labor mix. Keesler CBA currently determines wages for Housekeeing Aide and Group Leader. If offeror determines the need for other labor catogories the spreadsheet accommodates insertion of additional rows. Offeror will need to formulate the inserted row to ensure calculations are correct and to achieve a singular fully loaded wage rate for direct labor. Offeror may propose any combination of labor catergories to meet total clean requirements of the PWS.

18 Keesler IMFE – MEPRS BALA Surcharge for Dedicated RS #7.

Dedicated personnel to respond to the kitchen area within 5 minutes between the hours of 06:00 and 19:30. Personnel will not leave the area until work is completed. Work shall be started immediately after each meal is served and an “end-of-day” cleaning shall be performed prior to the area closing for the day. The BALA area is already under RS #7 as

9,025 SF.

Q: Please define what kitchen area cleaning tasks are required to be performed after each meal, and what additional tasks are required for end-of-day cleaning?

Q: Is the 5 minute response time for kitchen spills, etc. between after meal cleaning?

Q1: PWS provides requirement for aseptic cleaning.

Q2: Yes, response as required within the dedicated time frame as stated in the

IMFE.

Keesler IMFE - Staff Worksheet

Q: Surcharge for Kitchen Area cleaning indicates 12 occurrences per year, but the kitchen area is cleaned following three meals per day plus end-of-day cleaning 5-days per week. If the end-of-day cleaning is performed instead of after meal cleaning it would work out as follows

(unless there are 3 after meal cleanings plus EOD cleaning):

3 times (2 after meal + 1 EOD) x 5 days x 4.3 wks/mo. = 64.5 times/mo.

64.5 times/mo. x 12 months = 774 Occurrences per year.

What is actual number of surcharge cleaning occurrences per year?

Q: Exterior Windows box C26 is using the same number of SF as light fixtures and covers, but not all rooms in the SF listed have exterior windows. Need to know how many windows (or numbers of rooms that have windows) in order to determine and schedule with any realism how long it would take to clean them. How many windows and are they accessible from the inside above the ground floor for each installation?

If they are not accessible from the inside a window sub-contractor may be necessary to clean upper floors.

Q: Drapes box D29 hours per occurrence can not be filled in with any realism unless the number of drapes to be removed, cleaned, and re-hung is known. With no idea or estimation of how many drapes there are to clean the amount of time to perform the task would be a guess.

How many drapes per installation need to be cleaned twice per year?

Q1: The surcharge is to be invoiced/paid monthly, hence the 12 times per year.

The clenaing effort is performed daily IAW the IMFE.

Q2: The government does not have this information. A proposing contractor will be required to determine manhours based on expierence with buildings of like size, type, and use.

Q3: The contractor only removes and replaces (rehangs) the drapes. The contractor is not responsible for the actually cleaning of the drapes. The government has no information regards type, kind, and count.

20 Keesler IMFE indicates Linen Chute Cleaning is part of the contract.

Q: How many linen chutes are there?

Q: How often is the chute(s) cleaned?

Q: What tasks are required to clean the linen chute?

Q: The same questions above apply to all installations that have linen chute cleaning required.

Q1: Less than 5.

Q2: As needed to assure asepsis.

Q3: Open door, wipe down within arms length with a disinfectant.

Q4: Only Keesler has linen chutes.

21 Subfactor 1.1 bullet para. 5 – States…To include products, disposal of products, and nergy efficient equipment.

Subfactor 1.3 para. 5) and 6) requiring providing list of equipment and supplies to be used.

Q: Are contractors to provide the list of equipment and supplies for both subfactors, or just as part of the MFAM?

Subfactor 1.1 is to describe the management approach to environmentally-preferred products, overall. Subfactor 1.3 is specfic to the sample task.
