1 With regards to RFP # FA8052-12-R-0004.
Paragraph # 13 of the Keesler IMFE
13. LINEN SERVICES, 7 DAYS PER WEEK. (REF PWS PARA
2.1.20)
**Linen chute cleaning is a part of this contract.
Can the Government clarify the sentence that reads Linen chute cleaning is a part of this contract.
Does this mean that we would require a Linen Chute cleaning sub-contractor or does this imply general aseptic cleaning of the linen chute areas that can be reached by the housekeepers using their tools.
The requirement to clean Keeslers's linen chute is to clean the outside and the inside by reaching in and wiping down the interior chute as part of the total clean requirement. The Government does not determine whether the offeror will use a subcontractor to perform this task.
2 On page 236 of 246 of the file titled “pd28.pdf” (the basic solicitation), under Section L, the specific instructions for Part I – Administrative, paragraph 1.b states, in part, “A matrix shall be included cross referencing each paragraph of the PWS to the specific page and paragraph of the proposal that addresses it.” Typically, a proposal is structured with headings and subheadings being numbered, not every single paragraph. Is listing the page and section (heading or subheading number) of the proposal where each paragraph of the PWS is addressed sufficient? Or must every single paragraph be numbered?

3 On page 236 of 246 of the basic solicitation, under Section L, for Part II
– Technical, the instructions state “The pages shall contain the Offeror’s
Commercial and Government Entity Code (CAGE) and the Data
Universal Numbering System (DUNS) on the top right corner of each page of each copy of the technical proposal for identification purposes.”
Is the offeror also permitted to identify itself by name?
No, Offeror is not permitted to identify itself by name on the top right corner of each page of the copy of the technical proposal.
4 Should the offeror include the CAGE and DUNS in the upper right-hand corner of the one copy of the MFAM that is to be submitted?
(Page 236 of 246, basic solicitation, Part II – Technical, “The Medical
Facility Adaptive Manual (MFAM) is excluded from the 60 page limit.
Please submit only one copy of the MFAM”.)
Offeror does have to include CAGE and DUNS in the upper right-hand corner of the one copy of the MFAM.
5 Should the CAGE and DUNS be included in the top right-hand corner of all volumes of the proposal, or only for Volume II – Technical?
CAGE and DUNS are to be included only in Volume II Technical proposal.

6 5. On page 236 of 246 of the basic solicitation, under Section L, for
Part II – Technical, the instructions state “PART II – TECHNICAL -
Proposal shall be limited to 60 pages*… The Medical Facility Adaptive
Manual (MFAM) is excluded from the 60 page limit.” Are organizational elements – such as the table of contents, title/cover pages, and section tabs - also excluded from the 60 page limit?
No, organizational elements – such as the table of contents, title/cover pages, and section tabs are not excluded from the 60 page limit.
7 On page 242 of 246 of the basic solicitation, under Section L, paragraph
F.5.d states “Typing shall not be less than 12 pitch.” May informational tables (such as equipment/supplies listings, staffing summaries, etc.)
and graphics (such as organization charts or process charts) be smaller than 12 pitch?
Typine shall not be less than 12 pitch.
8 May the headers (e.g., the identifying CAGE and DUNS) and footers
(e.g., page number) be smaller than 12 pitch?
Typine shall not be less than 12 pitch.
9 The individual MTF spreadsheets are not in 12 pitch. Should the offeror reformat them to 12 pitch for submittal, or leave the formatting as it was provided by the government?
Offeror should not reformat the IMFE spreadsheets provided by the
Government.
10 PWS paragraph 3.2 refers to “Technical Exhibit D, Performance
Requirements Summary.” Where is this exhibit located?
Technical Exhibit D does not exist. The last sentence of paragraph 3.2 should be discounted. Paragraph 3.2 should read as: 3.2 Quality Control Program.
The Contractor shall maintain a quality control program that ensures “green” environmental services are performed in accordance with commonly accepted commercial practices, voluntary consensus standards, and in compliance with federal, state, installation, and MTF policies, procedures, and regulations as identified in this PWS. The Contractor shall provide procedures to identify, prevent, and ensure non-performance and continual repeat of defective service does not occur.
11 How does the Performance Requirements Summary referenced at PWS
3.2 differ from the Services Summary (SS) provided at PWS 2.2?
See response to #10 above.
12 PWS paragraph 7 lists two exhibit As: PWS paragraph 7.1 refers to
“APPENDIX A. Health Insurance Portability & Accountability Act
(HIPAA) of 1996” and PWS paragraph 7.4 refers to “EXHIBIT A.
Individual Medical Facility Exhibit (IMFE).” Please clarify.
Appendix A and Exhibit A are two different documents.
13 On page 45 of 45 of the PWS, “Exhibit A. Individual Medical Facility
Exhibit (IMFE)” is blank. Please clarify.
IMFEs are identified in Exhibit 2.
14 Will there be available office space provided from the government for the Management at each site?
Yes
15 Linen- What is the inventory of linen? Will the contractor be responsible for replenishing additional linen?
Winning Offeror at each site should accomplish a linen inventory. Offeror is not responsible for additional linen items.
16 Will there be washer/dryer units on site? If so, will this be provided by the government?
Washer/dryer units are not available and will not be provided.
17 Are there any inpatient/outpatient reports available for all Medical
Treatment Facilities?
No
18 Is there a list of approved subcontractors for each site? No

19 MFAM- per PWS page 32, Section 2.1.18- Submit no earlier than 20 calendar days after award but no later than start of performance at a particular site. And later in Solicitation page 238 of 246- states to submit all MFAM elements in 2.1.18. We are confused as to when this should be submitted; please clarify if the MFAM is to be submitted in technical or after award.
Only the MFAM for the sample task is to be submitted with the initial submission. Offeror(s) who are awarded task orders must submit the MFAM as directed in the PWS
