SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
The purpose of this amendment is to change the due date for Past Performance Information. See Section L, Paragraph 1.4.6.
SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
SABER SOLICITATION INFORMATION
INSTRUCTIONS TO OFFERORS
1.0 SPECIFIC PROPOSAL PREPARATION INSTRUCTIONS:
1.0.1 All prepared documents must be in a font no less than 10 points and have no less than 1 inch margins.
1.0.2 The binders or volumes described in the following will be submitted in an original and 3 (three) copies. The original and copies shall be clearly marked on the face of each volume/binder.
1.1 Executive Summary Section: Each offeror shall submit an Executive Summary of no more than 3 pages total which summarizes the key points from the offeror’s Mission Capability, Past Performance, and Price areas. The Executive Summary Section is not included in the Volume I page limitation.
1.2 Contract Forms: Include the following data in the format indicated. Designate on the cover, which proposal package contains the original or an executed copy of the proposal (RFP). These documents do not count toward the Volume I page limitation.
Section 1: Proposal forwarding letter, if any.
Section 2: A complete copy of this RFP (through Section K, less attachments) with the original signature of an official authorized to contractually bind the offeror in block 20B of the SF 1442. The offeror must complete the following sections within the RFP:
Part I, Section A (SF 1442, Blocks 19-20C).
Part I, Section B (Section B Supplement with proposed coefficients entered).
Part I, Section G – Persons Authorized to Negotiate or Conduct Discussions
Part IV, Section K, Representations, Certifications, and other Statements of Offeror. In addition to the on line representation and certifications the provisions at 52.204-8, 52.219-28 and 252.247-7022 will need to be completed and submitted. (See directions for Online Representations and Certifications Application (ORCA) in section K)
Section 3: Any exceptions to or deviations from, any of the clauses or special contract requirements along with justification therefore.
Section 4: Any information, excluding price, which does not properly belong within the other proposal volumes.

1.3
FACTOR 1
MISSION CAPABILITY TECHNICAL/ RISK
VOLUME I
At a minimum, the submission shall be prepared in a form consistent with the instructions described within this section and the evaluation criteria for award set forth in Section M of this solicitation. All information the offeror deems appropriate to address these criteria should be included in the proposal in an orderly format following the evaluation criteria to include factors and subfactors. To facilitate evaluation, Volume I should be specific, detailed, and clearly demonstrate that the offeror has a thorough understanding of the requirements for the accomplishment of this effort. Statements that the offeror understands, can, or will comply with the Specifications and phrases such as “standard procedures will be employed” or “well known techniques will be used” etc., will be considered unacceptable. The following technical subfactors comprise the Mission Capability Technical/Mission Capability Risk (Volume I) portion of the technical proposal:
1.3.1 SUBFACTOR 1 - Project Management Ability
1.3.1.1 Offerors shall address their management organization, to include a description of functions, authorities and credentials of key management personnel fulfilling at a minimum the personnel requirements specified with the contract specifications section 01000 paragraph 1.2.3 (Project Management, CAD/GIS Support, Site Superintendence, Quality Control, subcontract purchasing and administration and general office administration). Offerors shall address their plan for acquiring qualified subcontractors. The methods and criteria described must include consideration of subcontractor’s previous experience, present capabilities, and quality of previous jobs to ensure effective management of subcontractors and ensure quality workmanship and timely completion. Offerors shall address the proposed support and interface with the company or home office, initial startup/management of contingency situations, quality control plan, response and completion of warranty calls and punch list items, and overall management ability to coordinate multiple subcontractors and multiple projects.
1.3.1.2 Key Project Management Staff: Offerors shall provide an organizational chart describing functions, responsibilities and authorities performing such duties as overall project management, project engineering, CAD support, site superintendence, quality control, and subcontract purchasing and administration. The proposal shall specify hiring criteria (skill levels, experience, and background) for personnel in these positions. Offerors shall provide sample resumes for the type of personnel the offeror considers qualified for key positions to include background, education and experience. Qualified personnel may be able to hold more than one of the required positions. The project manager shall have the services of a Professional Engineer available for project review and certification of drawings when required.
1.3.1.3 Home Office and On-Site Support/Workload Management: The offer shall clearly define on-site support and interface with home office or corporate headquarters for such aspects as management, technical support, minimal design effort and financial support. The offer will demonstrate the ability to execute a large number of multi-discipline delivery orders, demonstrate how staffing and resources will be acquired to meet peak workloads and describe a strategy for executing a large number of delivery orders in a highly compressed timeframe as a result of cyclic nature of Government funding resulting in contract award late in the fiscal year.
1.3.1.4 Response and Completion of Warranty Calls and Punch List Items: The offer will identify and describe plans for warranty of construction as it applies to routine, urgent and emergency calls, as well as punch list items. The plan shall support response and correction completion time frames, personnel responsible for responding to such calls and performing corrections, and provisions for non-standard hour calls.
1.3.2 SUBFACTOR 2 Quality Control Plan
1.3.2.1 Quality Control Plan: The offeror shall provide a summary that will address all aspects of quality control to include responsibility for surveillance of work by both subcontractors and in-house quality control inspectors. The offer will describe the process for acceptance, rejection, documentation and resolution of deficiencies for work performed by the in-house work force or subcontractors, and corrective action to identify poor performance by any work staff. In addition, the Quality Control Plan shall discuss how the inspectors and/or superintendents will be qualified to supervise various trades to ensure quality workmanship.
1.3.3 SUBFACTOR 3 Project Development/Planning
1.3.3.1 This Subfactor will address the Offeror methods and procedures for project development and planning. This Subfactor will cover the development of a plan/checklist for covering these procedures and a plan/checklist for the major items to be included in each minimal design package.
1.3.3.2 Project Development and Planning: The offer shall identify methods and procedures for scheduling work to ensure performance periods are not excessive. Identify a plan and methods to track projects and interface with Government planners and inspectors. Identify a plan for the upkeep of a submittal log throughout the duration of the contract. Identify a plan and specify time for running work clearance requests permits for work on the installation. Identify a plan, contents, and time frame for updating property records and closing out projects
1.3.4 SUBFACTOR 4 Minimal Design and Completeness of Seed Projects:
1.3.4.1 The offeror shall provide a proposal cost breakdown worksheet in Microsoft Excel format of the in R.S. Means© line items selected to complete the seed project as defined in the Scope of Work for the project DEQ000878J, Extend Compost Awning, Sequalitchew Training Area, Landfill No. 5 Building PO7601. Submit one design concept for the seed project which includes a technical description, execution narrative, and engineering calculations to support the design. Include a list of assumptions used to develop concept(s), design drawings, material submittals, a progress schedule and a walk through of the development of performance period.
1.4 FACTOR 2 PAST PERFORMANCE VOLUME II
1.4.1 Contents of Past Performance Volume:
The offeror shall submit a Past Performance Volume containing the following:

Summary Page describing the role of the offeror and each subcontractor, teaming partner, and /or joint venture partner that the offeror is required to provide Past Performance Information Sheets on in accordance with paragraph 1.4.2 below.
Organization Structure Change History—See paragraph 1.4.5 below.
Past Performance Questionnaire in accordance with Attachment 6, Section J. Past Performance Information and Past Performance Questionnaire—See paragraph 1.4.6. below. Past Performance Questionnaires received from third party evaluators do not count toward the Volume II page limitation.
1.4.2 Past Performance Information Sheets: The offeror shall submit 3 to 5 Past Performance Information Sheets each for the prime contractor and/or teaming/JV partner identifying active or completed contracts, either Government or commercial, for each prime, teaming partner, and/or joint venture partner. Data presented must include only relevant contracts held within the past five (5) years and demonstrate an ability to handle multiple projects with multiple disciplines. The following information must be provided for each contract identified on the Past Performance Information Sheets, limited to two (2) pages per contract described:

Offerors are cautioned that the Government will use data provided by each offeror in this volume and data obtained from other sources in the evaluation of past and present performance.
1.4.3 Relevant Contracts: Relevant past performance information must demonstrate non-complex requirements (up to 35% design) with multiple projects being performed simultaneously involving multiple disciplines, such as construction, renovation, alteration and repair, with multiple disciplines, along with specific work in heating, ventilation and air conditioning (HVAC), electrical, mechanical and other major trades. Projects should be similar in size, scope, and complexity as those anticipated under this contract, i.e., having a project value between $2,000 and $750,000, and include projects with the Federal Government, state and local government agencies, and commercial customers.
1.4.4 Provide data for up to 5 relevant contracts held within the last five years for each entity the offeror wants the Government to consider. Performance Information Sheets for each contract is limited to 2 pages. Include rationale supporting your assertion of relevance. Note that the Government will not consider performance on an effort that concluded more than 5 years prior to the issuance date of this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate. To clearly link the past performance information to the mission capability subfactors, the offeror should identify which contracts are relevant indicators of performance against a mission capability subfactor.
Categorize the relevance information into the specific evaluation subfactor(s) used to evaluate the proposal:
Subfactor 1 – Project Management Ability
Subfactor 2 – Quality Control Plan
Subfactor 3 – Project Development/Planning
Subfactor 4 - Minimal Design and Completeness of Seed Project
1.4.5 Specific Content: Offerors may include a discussion of efforts accomplished by the offeror to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. Merely having problems does not automatically equate to a Little Confidence or No Confidence rating, since the problems encountered may have been on a more complex program, or an offeror may have subsequently demonstrated the ability to overcome the problems encountered. The offeror is required to clearly demonstrate management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified. This may allow the offeror to be considered a High Confidence or Significant Confidence candidate. For example, submittal of quality performance indicators or other management indicators that clearly support that an offeror has overcome past problems is required.
1.4.5 Organizational Structure Change History: Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
1.4.6 Past Performance Questionnaires: The offeror shall send out Past Performance Questionnaires to each of the Points of Contact (POCs) identified in the Past Performance Volume. In turn, the evaluating agencies shall submit the completed questionnaire directly to Mr. Terry Seaverns at Terry.Seaverns@mcchord.af.mil AND Mr. Clifford Jackson at Clifford.Jackson@mcchord.af.mil or by facsimile to (253) 982-3656 no later than May 25, 2010. The foregoing changed date is the result of late Government responses to potential contractor's questions concerning Past Performance Information. Those potential contractors that have provided Volume II of the proposal or have their Volume II in route may provide updates to Volume II by the above due date. However, updates must be accompanied by a cover sheet indicating where in Volume II the updates should be filed along with an original and three (3) copies of the updated information provided. No questionnaire submittals will be accepted from the offeror being evaluated. Once the evaluator completes the questionnaires, the information shall be considered sensitive and shall not be released to the offeror. The desired questionnaire that is to be transmitted to each point of contact is provided in Section J, Attachment 6.
1.4.7 Subcontractor Consent: Past performance information pertaining to a subcontractor, Joint Venture Partner or Team Member cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide a consent form all subcontractors, JV Partners or Team Members that will perform major or critical aspects of the requirement, consenting to release of their past performance information to the prime contractor. A sample consent is provided as Attachment 7 in Section J.

FACTOR 3
PRICE
VOLUME III
1.5.1. Price/Cost: The price/cost proposal shall consist of separate coefficients as solicited in the Schedule B. Offerors shall explain the basis for development of the coefficients. The explanation shall, as a minimum, address offeror’s perception of the accuracy of the unit price book (RSMeans©), and the basis for any assumptions regarding expected volume (dollars) of work under the contract and a list of the components of cost that were used in developing the proposed coefficients. The list of cost components will be used by the evaluation team to be sure all costs that could be reasonably incurred are included in the proposed coefficients.
1.5.2. Bonds: Offeror shall provide a letter from a surety attesting to bonding capability. The actual proposal bond for the initial task order shall be included in the initial task order proposal.
1.5.3. Financial Resources: The offer shall address the fiscal plan for covering phase-in, unexpected and/or emergency situations, and initial performance costs prior to recoupment in the form of progress payments. It is expected the offeror will need to cover an initial cash float (60-90 days) until such time as an adequate number of delivery orders can be issued to provide reimbursement of these initial expenses.
