Regional Kitchen / Finishing Kitchen Traceability & Recall Procedure Interim turnover baseline for supplier/farm-to-receipt-to-production-to-school traceability, product hold, recall support, reconciliation and closeout
CONTROL STATUS
INTERIM STATE BASELINE - NOT A FINAL APPROVED RECALL POLICY. This document provides the traceability and recall operating framework required for Regional Kitchen turnover. The Operational Support contractor is expected to develop/refine the final traceability and mock-recall system, and applicable State/DOH direction governs actual notification, recall authority, food disposition and regulatory reporting.
1. Purpose and Scope This procedure establishes the minimum information and actions needed to trace food through the Regional Kitchen/Finishing Kitchen operating model and to support a product hold, withdrawal, recall, foodborne-illness review, or other traceability event.
Trace product backward from a school/menu/date or RK batch to the supplier or farm, product/lot, receipt, storage and ingredient usage.
Trace product forward from a supplier lot, RK batch or product identifier to every Regional Kitchen production use, route and Finishing Kitchen/school destination.
Maintain quantity and final-disposition records so affected product can be reconciled rather than merely located.
Link traceability to SAFE-02 food-safety records, LOG dispatch/receiving records, menu/recipe records, and the branch foodborne-illness response process.
Use SAFE-04 to validate this procedure through mock-recall and incident exercises before full implementation.
2. Required Traceability Chain
Trace Stage
Minimum Identifier / Record
Primary Evidence
Why It Matters
Supplier / Farm / Source
Supplier/farm name; product; vendor item/SKU if used; lot/batch or other traceable identifier
Approved supplier/product record; receiving document
Establishes the origin of the affected product.
Regional Kitchen Receipt
Receipt date/time; quantity/unit; lot/batch; receiving condition; storage destination
SAFE-02 RK Receiving Log / source receiving record
Connects source product to RK inventory.
RK Storage / Inventory
Storage location; dating; inventory identity; transfer/use record as applicable
Storage/inventory record
Preserves product identity before production use.
Recipe / Production Batch
RK recipe/menu item; production date; batch/run identifier; ingredient-lot relationship
Production record; recipe/batch documentation; SAFE-02 logs
Connects ingredient lots to finished product.
Packaging / RK Release
Finished-product/batch identifier; quantity; route/school destination; release record
SAFE-02 RK Release / Dispatch; LOG-02
Creates forward trace from production to delivery.

Maintains chain of custody during movement.
Finishing Kitchen Receipt
School; receipt date/time; batch/route; quantity; acceptance/discrepancy; storage/next step
LOG-03; SAFE-02 FK Receiving Log
Confirms which school actually received the affected product.
FK Finish / Service
Rethermalization/holding/service record; service date/time; menu item; remaining product where applicable
SAFE-02 FK logs; production/service record
Connects delivered product to the meal-service event.
Final Disposition
Served, on hand, held, returned, discarded, or otherwise dispositioned quantity
Corrective-action/disposition record; SAFE-04 reconciliation
Shows whether all affected quantity is accounted for.
3. Traceability Record Rules Use stable identifiers. A trace must not depend on staff memory, a filename alone, or an informal description such as 'Tuesday chicken.' Do not overwrite an identifier after product has moved. If an identifier changes at production or packaging, preserve the relationship between the old and new identifiers.
Record the destination actually receiving the product, not only the planned destination.
When one batch is divided among multiple routes/schools, retain enough quantity information to reconstruct distribution by destination.
When multiple source lots are combined into one production batch, record every applicable source lot relationship.
Keep held, rejected, returned or discarded product traceable until final authorized disposition is documented.
Completed food-safety, dispatch, receiving and corrective-action records are evidence and should remain available even when blank templates are later superseded.
4. Trace-Back Procedure
Step
Action
Minimum Outcome
1
Define the event
Identify the school/site, meal date/time, menu/product, complaint/issue or affected batch identifier.
2
Secure product and records
When applicable, place remaining product on HOLD/DO NOT USE under the approved food-safety procedure and preserve relevant records.
3
Locate FK receipt
Identify the FK receiving record, route, quantity, batch/lot reference and delivery time.
4
Locate RK dispatch
Link the school receipt to the RK route/load and final release record.
5
Locate RK production batch
Identify the finished-product batch/run, production date, recipe/menu item and applicable food-safety records.
6
Identify ingredient/source lots
Determine which ingredient lots/supplier or farm records fed the affected batch.
7
Review related controls
Retrieve applicable receiving, storage, cooking, cooling, holding, packaging, transport, rethermalization and service records.
8
Document gaps immediately
If a link cannot be proven, record the missing evidence and elevate it as a traceability/control gap.
5. Trace-Forward Procedure
Step
Action
Minimum Outcome
1
Select source identifier
Start with the supplier/farm product/lot, RK production batch, or other confirmed affected identifier.
2
Identify RK inventory/use
Determine receipt quantity, storage/location, quantity used, production batches and any remaining/on-hand product.
3
Identify all finished batches
List every finished-product batch/recipe/menu use associated with the affected source identifier.
4
Identify all route/destination records
Use RK release/dispatch and route records to identify every school/FK that received affected finished product.
5
Confirm actual school receipt
Verify delivery/acceptance and quantity through FK receiving records; do not assume planned delivery equals receipt.
6
Determine final status
For each destination, document quantity served, on hand, held, returned, discarded or otherwise dispositioned as applicable.
7
Reconcile quantities
Use SAFE-04 or an approved recall tool to identify unaccounted quantities and unresolved destinations.
8
Expand scope when evidence requires it
If records show additional lots/batches/destinations may be affected, update the trace scope under authorized direction.
6. Product Hold, Recall Support and Disposition
IMPORTANT
This procedure does not create independent recall authority or food-disposition criteria. In an actual event, the State's approved HACCP/SOP, contract/procurement requirements and applicable regulatory direction control who may initiate notifications, expand the affected scope, release product, return product or direct disposal.
HOLD / DO NOT USE: identify and segregate affected or potentially affected product so it cannot be served or mixed with available stock while status is unresolved.
Preserve identification: keep lot/batch/product/destination records attached to the held product or clearly cross-referenced.
Protect condition: maintain required storage/temperature controls while product is held unless authorized disposition requires otherwise.
Do not destroy traceability during disposal or return. Record product, quantity, reason, date/time, destination or disposal method, and approving role as required.
If product is released after review, record the basis/authority for release and update the affected quantity reconciliation.
7. Communication and Escalation Traceability information should support - not replace - the established incident/communication process.
Situation
Minimum Traceability Information
Communication Control
Foodborne-illness complaint involving RK food
School, meal/date, product, FK receipt, RK batch/route, ingredient/source lots when found, related records and other destinations
Use the existing branch Foodborne Illness Complaint Response process and SAFE-05 crosswalk.
Supplier/product quality or safety notice
Supplier/product/lot, RK receipt/on-hand quantity, production batches, destinations and disposition
Escalate through designated procurement/food-safety/leadership channels.
Internal food-safety deviation
Affected product/batch, process step, quantity, destination if distributed, corrective action and disposition
Use SAFE-02 corrective-action record and escalate per approved HACCP/SOP.
Mock recall / exercise
Simulated source/batch, trace records, quantities, destinations, communication test and gaps
Document in SAFE-04; do not create a real external notification unless separately directed.
8. Quantity Reconciliation and Closeout Account for the source or affected quantity using receipt, production-use, on-hand, distributed, returned/held, discarded and other documented quantities as applicable.
Unaccounted quantity is a traceability gap. Do not close the event simply because the named schools were contacted.
For every destination, document a final known status or explicitly identify it as unresolved.
Close traceability actions only after missing-record issues and required corrective actions are assigned, verified or formally transferred to continued State management.
Retain a final event summary showing the source identifier, affected batches, destinations, quantity reconciliation, disposition, notifications/communications, corrective actions and approving/review roles.
9. Mock Recall and Validation Conduct at least one mock recall before full implementation as required by the Operational Support scope.
Test both trace-back and trace-forward capability using real system records or controlled simulated identifiers.
Capture elapsed time, records located, destinations identified, quantity reconciliation, communication performance and any missing/ambiguous records.
Document deficiencies and corrective actions in SAFE-04 and update SAFE-01, SAFE-02, training or SOP products as needed.
Repeat the exercise when material gaps remain or the approved food-safety program requires additional validation.

12. Document Control and Supersession This file remains an interim turnover baseline until a final approved traceability/recall procedure is accepted.
When the Operational Support contractor or State issues an approved replacement, identify the new controlling document in the turnover binder and archive this interim version.
Do not discard completed traceability, mock-recall, incident, corrective-action or receiving/production records merely because the blank procedure/template is superseded.
Any change to menu, recipe, ingredient, supplier, equipment, process, distribution model or regulatory requirement that affects traceability should trigger review of this procedure and SAFE-01 change control.
Source Basis Used for This Interim Procedure This turnover procedure was structured from the Regional Kitchen Operational Support food-safety/traceability requirements, the Regional Kitchen turnover binder, and the existing School Food Services Branch Foodborne Illness Complaint Response process. Final regulatory requirements and State-approved procedures remain controlling.
