About this file

This is a Payment Bond form (AMS-Form-025) used by Brookhaven National Laboratory, managed by Brookhaven Science Associates, LLC under contract to the U.S. Department of Energy. The form is a template for contractors to submit payment bonds as required for federal contracts.

The form captures essential contract and surety information including the principal's legal name and business address, organization type (individual, partnership, joint venture, or corporation), contract identification details (name, date, and number), and the penal sum of the bond expressed in millions, thousands, hundreds, and cents. The principal and surety(ies) bind themselves jointly and severally to Brookhaven Science Associates and any successor contractor to ensure prompt payment to all persons providing labor, materials, or both for contract work. The form accommodates multiple corporate sureties (up to seven identified as Surety A through G) and individual sureties, with fields for liability limits, state of incorporation, and signatory information including corporate seals. Instructions specify that corporate sureties must appear on the Department of Treasury's list of approved sureties and act within their listed limitations; individual sureties must provide a completed Standard Form 28 (Affidavit of Individual Surety). The bond must be executed after the contract date, and any deviations from this form require written approval from the Procurement and Property Management Division Manager.
