About this file

This is a Bid Submittal Form for an indefinite quantity work order contract with Pinellas County, Florida, for infrastructure repair services. The contract encompasses reconstruction and repair work including concrete repairs, site cleanup, milling and resurfacing, landscape work, fencing, sidewalk repairs, American with Disabilities Act (ADA) ramps, sheet pile installation, and incidental related items. The work is categorized into five task types: Non-Secure Areas during Normal Working Hours, Non-Secure Areas during Other than Normal Working Hours, Secure Areas during Normal Working Hours, Secure Areas during Other than Normal Working Hours, and Non Pre-priced Tasks. Each work order will be a standalone project with specific time limits and requirements as outlined in individual Job Order documents.

Bidders submit a single Adjustment Factor applicable to all unit prices in the Construction Task Catalog, with factors specified to four decimal places and subject to defined hierarchy constraints. The adjustment factors are weighted for bid evaluation purposes at 60% for Non-Secure Areas Normal Hours, 10% for Non-Secure Areas Other Hours, 10% for Secure Areas Normal Hours, 10% for Secure Areas Other Hours, and 10% for Non Pre-priced Tasks. Minimum adjustment factor thresholds apply, including that all adjustment factors must meet or exceed specified relationships (such as overtime rates being greater than or equal to normal rates within the same security classification, and secure area rates being greater than or equal to non-secure area rates). The County reserves the right to correct arithmetic errors and will calculate extended totals and the Award Criteria Figure using submitted adjustment factors.

TO THE COUNTY OF PINELLAS, FLORIDA:

We, the undersigned, hereby declare that no person or persons, firm or corporation, other than the undersigned, are interested in this proposal, as principals, and that this Proposal is made without collusion with any person, firm or corporation, and we have carefully and to our full satisfaction examined the Contract Documents, and that we have made a full examination of the location of the proposed work and the source of supply of materials, and we hereby agree to furnish all necessary labor, equipment, and materials, fully understanding that this is an indefinite quantity Contract, and that we will fully complete all necessary work in accordance with the Contract Documents, within the time limit specified in each Job Order for the unit prices set forth in the Construction Task Catalog®, multiplied by the Adjustment Factors inserted below.
