About this file
This is a Certification of Disbursement of Previous Periodic Payment to Subcontractors form required by the State of Florida Department of Transportation for construction projects governed by Florida Transportation Code Section 337.11. The form serves as a monthly certification that prime contractors must submit to verify all subcontractors have received their proportionate share of payments from the Department. The certification must be completed by an authorized officer or director of the contractor, notarized, and submitted to the Project Engineer no later than the Friday before the monthly estimate cutoff date (typically the third Sunday of each month) to avoid delays in payment processing. The form requires identification of any subcontractors not yet paid, along with documentation of good cause notifications sent to each unpaid subcontractor. The term "subcontractor" encompasses not only traditional subcontractors but also suppliers, service providers, and equipment rental firms whose materials, services, or equipment are incorporated into the work or stockpiled at the project site for which partial payment has been made.
False statements or omissions in this certification can result in suspension or revocation of bidding qualification, determination of non-responsibility status, and exposure to civil and criminal penalties under applicable Federal and State law. The form includes fields for contract number, financial project identification, contractor name, subcontractor details, notary information, and certification date. For the referenced S. Apopka Avenue road resurfacing project in Citrus County, this certification document would be required monthly from the selected contractor to facilitate timely payment processing throughout the project's anticipated 120-calendar-day completion window.
