PAST PERFORMANCE QUESTIONNAIRE
INSTRUCTION PAGE
Please complete the following questionnaire concerning evaluation of the Contractor’s past performance. Your response to this evaluation will be utilized for the procurement of a construction firm to provide all supervision, labor, materials and equipment necessary under a Mini-Multiple Award Construction Contract, Indefinite Delivery/Indefinite Quantity (ID/IQ) contract vehicle for construction services located primarily at Joint Base Elmendorf-Richardson
(JBER) AFB, AK.
Increased emphasis in our procurements is placed on past performance as a source selection factor. Your complete response is critical as this will be relied on by the Government in the selection of a firm for the proposed construction project. Please answer all questions by checking only one (1) response per question. It is strongly requested that additional information is placed in the space provided.
You will be required to sign this evaluation at the end of the questionnaire and your initials are required on each page of this questionnaire, as well as on this instruction page.
Government personnel may contact you concerning responses made by you in this questionnaire.
