About this file

This document is a Performance Evaluation Questionnaire for a federal contractor. It seeks feedback on the contractor's performance in areas such as timeliness, management, quality, and overall satisfaction. The questionnaire provides rating definitions and requests the evaluator to provide a rating and rationale for each performance area. It also asks whether the evaluator would recommend the contractor for similar requirements in the future.

The related federal contract opportunity is for the Petersburg Office Consolidation project in Petersburg, Alaska. The solicitation requires major interior renovation of the Petersburg District Office building, partial renovation of the Harris Building, and excavation/fill work at the Fram Compound. The solicitation provides details on a virtual pre-proposal meeting, the deadline for questions, and requirements for proposal submission. No substitution requests will be considered prior to award, and proposals must fully respond to the Technical Evaluation Factors to be considered further. The contracting agency is the Department of Agriculture Forest Service.
