About this file

The document provided is Attachment 6, the Affirmation of Support Form, related to a Request for Proposals (RFP) issued by the New York State Office of Children and Family Services (OCFS) to solicit applications to provide Healthy Families New York (HFNY) home visiting services with Healthy Families America (HFA) Child Welfare Protocols. The RFP targets 21 unserved counties in New York State, expanding the availability of HFNY services by allowing child welfare to refer families with children up to 24 months of age. The RFP specifies that approximately $9 million in annual funding will be available, with awards of up to $500,000 per county based on the number of families proposed to be served, generally equivalent to 10-15% of Medicaid births in the county(ies). The RFP has a due date of June 10, 2022 and a contract term of October 1, 2022 to September 30, 2027.

The RFP includes Minority-Owned, Women-Owned, and Service-Disabled Veteran-Owned business contracting goals. No more than one proposal per county will be approved for funding. The document also indicates that the RFP, Bidder's Notice, Attachment 7, and Questions and Answers have been amended, and additional information is available in the amended documents.
