About this file
This document is an attachment containing performance indicators for the Healthy Families New York (HFNY) program, which is a home visiting service supported by the New York State Office of Children and Family Services (OCFS). The performance indicators cover areas such as quarterly performance targets, retention rates, assessment and home visit timelines, accreditation requirements, supervisor observations, and program capacity. The document also references a related state and local contract opportunity, RFP # 1078, which solicits applications to provide HFNY home visiting services, including the Healthy Families America (HFA) Child Welfare Protocols, to unserved counties in New York State. This RFP expands the availability of HFNY services by enrolling children up to 24 months of age when referred by child welfare and adding programs in 21 unserved counties. Approximately $9 million annually will be available to fund proposals, with awards of up to $500,000 per county based on the number of families to be served, which should generally be equivalent to 10-15% of Medicaid births in the county(ies). The contract term is from 10/1/2022 to 9/30/2027, and the submission deadline is 6/10/2022.
The RFP includes a Minority Owned Sub-Contracting Goal of 15%, a Women Owned Sub-Contracting Goal of 15%, and a Service-Disabled Veteran-Owned Business Contracting Goal of 6%. No more than one proposal to serve each county will be approved for funding, and the applicant's proposal should propose to serve the equivalent of up to 15% of Medicaid births in their county(ies).
