About this file
This document is the revised Attachment 4 - Performance Indicators for the Targeted Healthy Families New York (HFNY) Program with Healthy Families America (HFA) Child Welfare Protocols RFP issued by the New York State Office of Children and Family Services (OCFS). The performance indicators cover areas such as quarterly performance targets, retention rates, assessment completion, home visit timing, required forms, staff training, and supervision. The RFP seeks applications to provide HFNY home visiting services, including the HFA Child Welfare Protocols, to unserved counties in New York State. A total of approximately $9 million annually will be available to fund proposals to serve the targeted counties, with annual awards of up to $500,000 per county based on the number of families to be served, which should generally be equivalent to 10-15% of Medicaid births in the county(ies). The RFP targets the counties of Cayuga, Chautauqua, Columbia, Essex, Fulton, Genesee, Greene, Hamilton, Lewis, Montgomery, Nassau, Onondaga, Putnam, Saratoga, Schoharie, Schuyler, Seneca, Tompkins, Warren, Washington, and Wyoming. The proposal submission deadline is June 10, 2022, and the contract term is October 1, 2022 to September 30, 2027.
The RFP includes Minority and Women-Owned Business Enterprise (MWBE) subcontracting goals of 15% each, as well as a 6% Service-Disabled Veteran-Owned Business (SDVOB) contracting goal. No previous identical or substantially similar procurement contracts have been awarded within the past five years.
