About this file

This document is a Joint Additive Manufacturing Acceptability (JAMA) IV Industry Vendor Capability Questionnaire designed to assess and integrate industry vendors into the Department of Defense (DoD) additive manufacturing (AM) supply chain. The questionnaire comprehensively evaluates vendor capabilities across multiple dimensions, including required federal certifications (CAGE Code, SAM.gov, NIST 800-171), manufacturing readiness levels (MRLs), production experience, quality management standards (ISO9001, AS9100, NADCAP), and specific additive manufacturing capabilities for both metal and polymer 3D printing technologies.

The survey seeks detailed information about vendors' technical capabilities, including 3D printing services for various materials like IN718, Ti64, AlSi7Mg, and polymer materials such as ABS, PLA, and aerospace-grade polymers like PEEK. It covers post-processing services, international trade regulation compliance (ITAR), supply chain security measures, and preparedness for potential participation in the JAMA IV pilot parts program. The questionnaire is part of a collaborative effort involving the Defense Logistics Agency (DLA), Office of Secretary of Defense, National Institute of Aviation Research, and various Military Services to expedite AM adoption in the DoD supply chain, with findings to be anonymously summarized and shared with participants and stakeholders.

Exhibit A – Joint Additive Manufacturing Acceptability (JAMA) IV Industry Vendor Capability Questionnaire

The JAMA project is a collaboration across the Defense Logistics Agency (DLA) Research & Development (R&D), the Office of Secretary of Defense (OSD) Research and Engineering (R&E), the National Institute of Aviation Research (NIAR), and various Military Services (MILSVCs) with the goal of expediting the adoption of Additive Manufacturing (AM) throughout the Department of Defense (DoD) supply chain. JAMA IV specifically aims to integrate industry vendors into the AM supply chain by conducting trials for AM parts procurement and manufacturing, and facilitating quality assurance (QA) testing to ensure the adoption of reliable and high-quality AM components.

Your responses to the questions in this survey may not preclude you from participating in JAMA IV, please complete the survey accurately and honestly to assist in planning for JAMA IV part production. Upon completion of the questionnaire, the findings will be summarized and released to questionnaire participants and JAMA IV stakeholders, per DLA and OSD discretion. All information provided will be made anonymous and kept confidential upon the release of any findings.

Thank you in advance for your participation.
