About this file
This is a Past Performance and References Response Template for an Oregon State Lottery RFP (#S-177000-00016166) for Mobile App Replacement and Associated Technology Solutions. Proposers are required to submit three relevant projects completed within the past five years as evidence of past performance capability. Each reference submission must not exceed two pages and must be completed using the provided template structure without alterations. The template requires proposers to document project titles, client organization information, industry sector, project scope and complexity, start and end dates, the proposer's role designation (prime or subcontractor), percentage of project completion, technology and services provided, and key outcomes with quantifiable results when possible.
The reference submission process requires proposers to complete the template for each project, send it directly to the designated project reference contact at the client organization for accuracy review and concurrence, and have that reference contact submit the completed document directly to Oregon State Lottery Procurement Specialist Randy White at randy.white@lottery.oregon.gov. The email subject line must include the RFP number, project name, and reference designation. Reference contacts must be current employees of the organizations associated with each past performance project and provide current contact information including name, title, phone number, and email address.
