About this file
This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on the DPAA Papua New Guinea Helicopter Requirement (Solicitation HTC71126RE147). The questionnaire is completed by past performance references identified by offerors and submitted directly to the Contracting Officer at brenda.l.marshall10.civ@mail.mil.
The PPQ evaluates contractors across three primary dimensions: Scope, Magnitude, and Complexity. For Scope relevance, contractors must demonstrate execution of at least three of four rotary-wing aviation capabilities: aviation operations (passenger/cargo transport), mission planning collaboration, emergency response/MEDEVAC readiness, and operational contingency (substitute aircraft provision). For Magnitude relevance, contractors must meet or exceed three of four quantitative thresholds: supporting one to three distinct deployments annually, executing sustained operations of 14-90 consecutive days per deployment, simultaneously managing up to three primary aircraft with alternate capability, and demonstrating a past contract value of at least $5,000,000. For Complexity relevance, contractors must document execution under at least three of four challenging conditions: austere environments (remote, mountainous above 5,000 feet, jungle, island, or overwater OCONUS locations with variable weather), limited infrastructure requiring operations from auxiliary/remote locations, regulatory authorizations requiring foreign host-nation coordination for landing rights and airspace clearances, and austere logistics including external sling-load operations and remote fuel planning. References rate contractor performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) and provide narrative commentary on contract award recommendation.

SECTION III: PERFORMANCE QUALITY RATINGS
(Please check/X the box that most accurately describes the contractor's performance) Rating Definitions
Exceptional (E)
Performance exceeded contract requirements to your benefit. Problems were negligible and resolved effectively.
Very Good (VG)
Performance exceeded some requirements. Minor problems were resolved quickly.
Satisfactory (S)
Performance met all contract requirements. Satisfactory corrective actions were taken for minor problems.
Marginal (M)
Performance failed to meet some requirements. Serious problems occurred; corrective actions were only marginally effective.
Unsatisfactory (U)
Performance failed to meet most requirements. Serious problems; corrective actions were ineffective.
Not Applicable (N/A)
No basis of evaluation/not observed.
PART A: SCOPE
Scope. To be deemed relevant in Scope, the past performance effort must have included the execution of at least three (3) of the four (4) following rotary-wing aviation support capabilities:
1. Aviation Operations: Rotary-wing (helicopter) flight services executing both passenger transportation and cargo movement.
2. Mission Planning: Direct collaboration with client planners to develop, coordinate, and execute flight mission plans.
3. Emergency Response: Maintenance of active, on-call MEDEVAC or emergency response flight capabilities.
4. Operational Contingency: Direct execution of aircraft maintenance programs and the active provision of equivalent substitute aircraft to ensure zero mission interruption.
Evaluation Element
E
VG
S
M
U
N/A
A1. Quality of Aviation Operations (Passenger/Cargo)
A2. Collaboration in Mission Planning
A3. Readiness for Emergency Response (MEDEVAC)
A4. Effectiveness of Operational Contingency (Substitute Aircraft)
PART B: MAGNITUDE
Magnitude. To be deemed relevant in Magnitude, the past performance effort must meet or exceed at least three (3) of the four (4) following quantitative operational thresholds, which represent the baseline scale of this requirement:
1. Mission Frequency: Supporting one (1) to three (3) distinct operational deployments/missions per year.
2. Mission Duration: Executing sustained flight operations where each deployment/mission lasted between 14 and 90 consecutive days.
3. Fleet Scale: Simultaneously supporting and operating up to three (3) concurrent primary aircraft (including both single- and twin-engine configurations), along with designated alternate/substitute aircraft capability.
4. Financial Value: The past performance reference had a Total Contract Value (including all options) of at least $5,000,000.

B1. Management of Mission Frequency
B2. Mission Duration
B3. Fleet Scale Support – Sustain Flight Ops
B3. Management of Overall Contract Value/Scale
PART C: COMPLEXITY
Complexity. To be deemed relevant in Complexity, the past performance effort must have been executed under at least three (3) of the four (4) following challenging geographic, logistical, and regulatory conditions, defined as contracts requiring:
1. Austere Environments: Flight operations conducted in remote, mountainous (elevations exceeding 5,000 feet), dense jungle, island, or overwater OCONUS environments subject to rapidly changing weather.
2. Limited Infrastructure: Executing flight operations and maintenance from auxiliary or remote locations with little to no established airport or ground support infrastructure.
3. Regulatory Authorizations: Direct coordination with foreign host-nation civil and military aviation authorities to secure landing rights, airspace clearances, and diplomatic permits.
4. Austere Logistics: Direct execution of complex external sling-load operations and remote fuel planning (obtaining, pre-positioning, and staging fuel) in close coordination with U.S. Government or Department of Defense mission personnel.
