About this file

This is a Business Compliance Checklist that serves as a mandatory cover sheet for proposal submissions to the DPAA Papua New Guinea Helicopter Requirement solicitation (HTC71126RE147).

The checklist establishes comprehensive compliance requirements across seven major categories. Offerors must submit a signed SF 1449 form with required blocks completed, including CAGE code and authorized signature, along with verification of active SAM.gov registration. Payment and remittance information must be verified, with confirmation that remittance address matches SAM.gov registration. Offerors must acknowledge receipt of all solicitation amendments via signed SF 30 forms and complete the RFP Offeror Information Sheet (Attachment 9) with company details, CAGE code, UEI, and authorized points of contact. Eligibility requirements include completion of the Basic Eligibility and Carrier Flag Status form (Attachment 8), provision of current FAA/CAA Operations Specifications or foreign equivalent and DoD CARB approval documentation, and submission of either active proof of insurance meeting PWS 4.15.2 limits or a signed written commitment from an authorized corporate officer to secure required coverages. Representations and certifications must be maintained as active in SAM.gov, with offerors selecting either full alignment or alignment with specified exceptions, including potential disclosures for foreign entity status and security prohibitions. Cybersecurity compliance requires confirmation of current DoD NIST SP 800-171 assessments uploaded in SPRS and active CMMC Level 2 Self-Assessment affirmation with the SPRS 10-character unique identifier provided. The checklist emphasizes that failure to complete all required items may result in proposal rejection and exclusion from further evaluation or award consideration.
