About this file
This is a Vendor Experience & Qualifications Questionnaire for an audiovisual (AV) system installation project. The questionnaire evaluates prospective vendors across three primary categories: vendor experience and qualifications, project management and approach, and training, support, and warranty capabilities. Vendors must demonstrate a minimum of three years in business and provide detailed information on at least three similar projects completed within the past three years. The scope of work requires comprehensive AV system design that meets specified functional requirements, including detailed user interface specifications, project timelines covering design through training phases, and quality control standards for on-site installation. Vendors must address scope change management, safety protocols, and the use of any subcontractors with documented qualifications. Additionally, vendors are required to submit drawings, signal flow diagrams, and ceiling reflection plans as part of their proposal.
The questionnaire mandates that selected vendors provide comprehensive training programs for staff and end-users upon project completion, along with complete documentation including user guides and as-built drawings. Warranty and service level agreements must be clearly defined, with specified troubleshooting response times for post-installation support. Vendors must affirm compliance with all federal, state, and local regulations and certify adherence to all scope of work requirements, terms and conditions, and insurance requirements as outlined in Section 4.08 of the draft contract. A Certificate of Insurance must be submitted as part of the proposal package. This questionnaire serves as the evaluation mechanism to determine vendor capability to execute the AV system installation project according to contractual specifications and performance standards.
