About this file

This document is a Contract Modification #2 and Extension #1 between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company to extend and modify an existing voluntary group disability insurance and benefits contract. The original contract was executed on January 18, 2022, under RFP #2020-06-35, and was previously amended on December 18, 2023, to add group legal services plan benefits. This extension modifies the contract for an additional 24-month term running from January 1, 2025, through December 31, 2026, with no further extension periods remaining. The extension covers voluntary long-term disability insurance, voluntary short-term disability insurance, and employer-paid short-term disability alternative plan options for Franklin County benefits-eligible employees, and adds pet insurance and optional identity theft insurance as new offerings.

The total not-to-exceed amount for this extension is $185,000.00. Renewal rates for voluntary long-term disability coverage show a 1.1 percent decrease across all age bands, while voluntary short-term disability rates reflect a 4.2 percent decrease. Metropolitan Life remains the incumbent contractor and will provide employer-paid group legal services plan benefits at a new participation fee of $8.25 per month, while the existing employee-paid legal services plan rate remains $16.50 per month. Pet insurance will be entirely employee-paid with rates determined by the specific plan selected. Identity theft insurance rates are guaranteed for three years and offered at varying monthly rates depending on plan type and coverage level. The contractor's notice contact is updated to Charlie Cross, Senior Account Executive, located in Southfield, Michigan, and all policy discounts applicable to benefits-eligible employees will be applied to the contractor's rates.
