About this file

This is a Buy American Certificate template required for federal contract submissions. The document serves as a certification form that contractors must complete to affirm compliance with Buy American Act requirements when submitting proposals or bids to the federal government. The certificate is applicable to this Department of Veterans Affairs solicitation (36C25926Q0875) for PIV Entry Point Licenses Software and Install Assistance, which requires the furnishing of Entry Point Identity and Access Management software, devices, and installation services for the C-Cures S PACS system. Contractors using this template must certify that end products, materials, and components incorporated into the deliverables meet Buy American Act domestic content requirements, unless specific exemptions apply. The form requires identification of the contract number, contractor information, and detailed declarations regarding the country of origin for all materials and products being supplied, ensuring full transparency regarding domestic versus foreign sourcing in accordance with federal procurement regulations.
