About this file
The document is a Hazardous Materials (HAZMAT) Usage Worksheet and instructions for contractors working on Department of the Air Force Space Command projects at Space Launch Delta 45 installations. The worksheet requires contractors to submit comprehensive details about any hazardous materials they plan to use, including Safety Data Sheets (SDS), product trade names, manufacturer information, container types and sizes, and total quantities. Contractors must obtain prior approval from the 45 CES/CEIE Hazardous Materials Program Manager (Richard Blanton or Thomas Powers) before bringing any hazardous materials onto the installation, and must track and report material usage quarterly.
Specific requirements include separately identifying different coating types, container sizes, and providing detailed information for each material line item. Contractors must submit the completed worksheet to Mr. Blanton and Mr. Powers, with additional cc recipients, and comply with Air Force regulations such as AFMAN 32-7002 Environmental Compliance and Pollution Prevention. The worksheet will be tracked in the Air Force's EESOH-MIS data system, and contractors must update the document throughout the project, with a final updated submission at project completion. This process applies to all materials including paint, oil, grease, adhesives, and sealants used in the project.
