About this file

This document is a Past Performance Questionnaire (PPQ) related to a federal contract opportunity for Architect-Engineering (A-E) design services to construct a Hybrid Operating Suite at the San Francisco VA Medical Center.

The key details are:

  • The VA Program Contracting Activity Central (PCAC) is seeking A-E services for a project to construct a 5,800 SF addition to Building 200 at the San Francisco VA Medical Center, including three specialized operating rooms and renovations to the Post-Anesthesia Care Unit.
  • The anticipated contract award is in September 2024 with a 260-calendar day period of performance for the design.
  • The project is a 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB) with a $13 million design-within-funding limitation.
  • The PPQ form is to be completed by the client to evaluate the past performance of the offeror across criteria such as quality, schedule, communication, management, cost, and subcontract management. Offerors must incorporate completed PPQs into their SF 330 submission.
