Standard Form 330 - Architect-Engineer Qualifications Instructions and Form

This document is the Standard Form 330 (REV. 7/2021), a federal form used by federal agencies to collect professional qualifications information from architect-engineer (A-E) firms for federal contract opportunities. The form is prescribed by the General Services Administration (GSA) under the Federal Acquisition Regulation (FAR) and has OMB Control Number 9000-0157 with an expiration date of 1/31/2027.

The form consists of two primary parts. Part I captures contract-specific qualifications for a particular federal contract opportunity, requiring firms to provide contract information, A-E point of contact details, proposed team composition, organizational chart, resumes of key personnel, relevant project examples (up to five), and example projects best illustrating the team's qualifications for the contract. Part II collects general qualifications applicable to a firm or specific branch office, including solicitation information, firm establishment date, unique entity identifier, ownership structure, small business status, point of contact information, former firm names, employees by discipline, firm experience profile over the last five years, and annual average professional services revenues for the previous three years. The form instructions specify that firms must provide only the information requested by the agency, be concise, and clearly document all key personnel participation in example projects. Section E requires resumes for each key person detailing their name, role, total years of experience, years with current firm, education, professional registration, and other qualifications. Section F requires up to ten example projects with project details, owner information, and team member involvement. Section G is a matrix showing which key personnel participated in which example projects. An authorized representative must sign and date the completed form, certifying that all information is current and factual.
