EXECUTIVE SUMMARY
On December 17 and 18, 2019, ECS Midwest, LLC (ECS) performed an asbestos-containing materials survey of the property located at 500 East Veterans Street, Tomah, Monroe County, Wisconsin.
The subject property is currently developed with a three story with partial basement, approximately 38,365 square foot building (Building 404) which is part of the VA Medical Center campus. As we understand, the on-site structure is scheduled for renovation.
The results of our ACM sampling and analysis indicated that asbestos was detected in the following material:
• Black mastic associated with tan vinyl flooring, 2nd floor nurses station, ~ 300 SF
In addition, the following materials were presumed to contain asbestos:
• Transite Cement Board - Located in perimeter building walls behind convector heating coil units, below all exterior windows
• Tagged Rated Fire Doors
Prior to renovation activities, the above identified materials will need to be removed by a Wisconsin licensed asbestos contractor using methods in accordance with current applicable state and federal regulations. The work will also need to be monitored and confirmed complete by an asbestos professional. ECS can provide costs for removal and associated oversight upon request.
If suspect asbestos-containing materials are uncovered during future renovation of the structure, which were not accessible during this survey or were outside of the survey area, it is recommended that these materials be sampled immediately upon discovery for asbestos content by a licensed asbestos inspector and submitted to an accredited laboratory for analytical interpretation or assumed to be asbestos containing. In addition, please note that identified and/or presumed ACMs may be located within other areas of the building not previously identified. It is recommended that an abatement contractor determine the location and quantities of the identified and presumed ACMs prior to abatement activities. Our recommendations are based on the guidelines presented in USEPA, State of Wisconsin, and OSHA asbestos regulations.

1.0 SITE DESCRIPTION
The subject property is currently developed with a three story with partial basement, approximately 38,365 square foot building (Building 404) which is part of the VA Medical Center campus. As we understand, the on-site structure is scheduled for renovation.
2.0 PURPOSE
The purpose of the Asbestos Materials Survey was to identify asbestos-containing materials (ACMs), which may require special handling and/or disposal if removed during construction activities. The identification of ACMs may require trained labor, regulated work practices, and special disposal.
3.0 METHODOLOGY
The survey was performed by Mr. Jorge Barrera of ECS (Wisconsin DHS No. 247713). A copy of Mr.
Barrera's license is provided in Appendix I. The sampling was conducted in general accordance with the U.S. Environmental Protection Agency (USEPA) requirements as defined in the National Emissions Standards for Hazardous Air Pollutants (NESHAP) guidelines.
ECS first assessed the building areas to determine homogenous materials, which are groups of materials installed in the same general time period and are uniform in color and texture. Each homogenous material was classified in one of the following three (3) categories:
• Suspected to contain asbestos and requiring sample collection,
• Not suspected to contain asbestos and no samples required, and
• Assumed to contain asbestos.
ECS then collected samples from random locations for each of the suspect homogeneous materials identified.
3.1 Asbestos-Containing Materials
Sample methodology was conducted in general accordance with US Environmental Protection Agency (EPA) and Occupational Safety and Health Administration (OSHA) regulations. The samples collected were submitted to Eurofins CEI, North Carolina for analyses by polarized light microscopy (PLM) (EPA Method 600/R-93). The Lab participates in the National Voluntary Laboratory Accreditation Program (NVLAP), and is identified with NVLAP lab code 101768-0. Non-organically bound (NOB) materials (i.e., floor tile, mastic, roofing tar) that are non-detect or equal to or less than one percent (trace) asbestos were then analyzed via transmission electron microscopy (TEM) analysis (EPA Method 600/R-93). EPA recommended chain-of-custody procedures were maintained throughout the sampling and testing operations.
In general, the finishes/materials sampled were observed to be in good condition (i.e. non-friable).
The sample numbers, location of the collected samples, and asbestos content, are provided in Table 1, entitled “Summary of Asbestos Containing Materials Sample Results,” included in this report.

As noted in the table above, the results of our ACM sampling and analysis indicated that asbestos was detected in the following material:
• Black mastic associated with tan vinyl flooring, 2nd floor nurses station, ~ 300 SF
4.2 Suspect or Assumed Asbestos-Containing Materials
Due to the inaccessibility or the destructive means that asbestos sampling requires, additional suspect ACMs may remain within the building hidden behind inaccessible areas that include, but are not limited to, sub-grade walls, structural members, topping slabs, sub-grade sealants, flooring located below underlayments, areas behind exterior walls, pipe trenches, and subsurface utilities, etc. These areas were deemed inaccessible and were not assessed.
April 10, 2020
It should be noted that the survey was performed while the building was in use for medical staff and patients, and many areas were unavailable to be surveyed as access was restricted. Most of these areas appeared to contain homogeneous materials to those sampled. However, if additional materials not previously identified are uncovered, these materials should be presumed to contain asbestos and be treated as ACMs, or be sampled immediately upon discovery and prior to disturbance for asbestos content by a certified asbestos inspector in accordance with 29 CFR 1926.1101.
In addition, ECS was provided a ACM Spreadsheet Inventory and a partial Asbestos Inspection Report. The report assumes that asbestos is present in the following materials:
• Transite Cement Board - Located in perimeter building walls behind convector heating coil units below all exterior windows
• Tagged Rated Fire Doors - Original doors
5.0 RECOMMENDATIONS AND REGULATORY REQUIREMENTS
Based on our understanding of the purpose of the Asbestos Materials Survey, the results of laboratory analysis, and our findings and observations, ECS presents the following recommendations.
5.1 Asbestos-Containing Materials
In addition, please note that identified and/or presumed ACMs may be located within other areas of the building not identified in the above table. It is recommended that an abatement contractor determine the location and quantities of the identified and/or presumed ACMs prior to abatement activities. Our recommendations are based on the guidelines presented in USEPA, State of Wisconsin, and OSHA asbestos regulations.

Dear Customer:
Enclosed are asbestos analysis results for PLM Bulk samples received at our laboratory on December 20, 2019. The samples were analyzed for asbestos using polarizing light microscopy (PLM) per the EPA 600 Method.
Sample results containing >1% asbestos are considered asbestos-containing materials (ACMs) per EPA regulatory requirements. The detection limit for the EPA 600 Method is <1% asbestos by weight as determined by visual estimation.
Thank you for your business and we look forward to continuing good relations.
