About this file
This is an Asbestos Assessment Survey Report prepared by Cape Environmental Management Inc. for the Louis A. Johnson Veterans Affairs Medical Center in Clarksburg, West Virginia.
The survey assessed 12 selected buildings at the facility totaling approximately 354,982 square feet. Cape Environmental conducted a room-by-room visual inspection to identify suspect asbestos-containing materials (ACM), collected representative bulk samples for laboratory analysis via Polarized Light Microscopy (PLM), and prepared cost estimates for abatement and removal. The assessment methodology followed AHERA 40 CFR 763 criteria, with samples analyzed primarily by the St. Louis VA Medical Center's Asbestos Analytical Laboratory (113/JB), with less than 3% analyzed by Cape Environmental in Atlanta. The survey identified ACM in Buildings 1, 3, 4, and 7, including floor tiles, mastic, ceiling tiles, pipe fitting insulation, and piping insulation. Buildings 5, 6, 10, 14, 15, 17, and 18 tested negative for asbestos. The Grand Total Response Action Cost across all facilities is $151,859.00, with Grand Total Removal and Replacement Cost of $846,573.00. Building 1 (Main Hospital) represents the largest cost burden at $77,961.00 for response actions and $667,100.00 for removal and replacement. The report includes detailed cost breakdowns by floor, room inventory databases in Microsoft Access, and recommendations for monitoring or removal based on material condition and accessibility.
