This document is an Offer and Cover Summary form for construction bids submitted to Salt Lake County, Utah for the Midvale Campus Renovation project (FAC133816-1). Bidders must provide their legal business name, address, contact information, and certify that submitted bid information is accurate and complete. The solicitation is invitation-only and limited to three pre-approved Stage 1 contractors: Layton Construction (ICS), SIRQ Construction, and Willmeng Construction. The project involves renovating the existing Midvale Campus building at 662 W Commerce Park Drive and constructing a 40-plus space parking structure adjacent to the building to relocate County operations. Bids were due November 19, 2026, and bidders were required to attend a mandatory site visit. The contract term is 365 calendar days from Notice to Proceed, with liquidated damages of $1,000 per calendar day for schedule delays. Award will be made to the lowest responsive and responsible bidder whose bid falls within the construction budget, contingent upon funding approval by the Mayor. Bids remain valid for 60 calendar days after opening.

Bidders must be registered Utah contractors holding a minimum B-100 license and certify compliance with all project documents including plans, specifications, and general conditions. A 5% bid bond on AIA Form A310-2010 is required with initial submission, and performance and payment bonds at 100% of the contract price are due within ten days of award. Pricing must include a base bid and alternates (Pedestal Pavers, Secure Parking Heat, and Alternate #4), with all costs covering materials, labor, equipment, and work completion in a workmanlike manner. Environmental and pollution liability coverage of $10 million specific to the Midvale Slag Superfund site remediation is mandatory. Minimum insurance requirements include $5 million general liability and $10 million commercial automobile liability. Projects exceeding $3 million must comply with County Ordinances 3.26 (apprenticeship requirements of 10% apprentice labor hours per trade with penalties of $2 to $10 per unmet hour) and 3.27 (construction procurement standards). All subcontractors must hold licenses appropriate to their work. Campaign contributions are limited to $100 per candidate under County Code 2.72A. Funding is contingent on approved funding levels and mayoral approval.
