TACCOM II PAST PERFORMANCE RATING FORM
PAST PERFORMANCE RATING FORM
INSTRUCTIONS TO OFFERORS SENDING PAST PERFORMANCE RATING FORM
Section A, PAST PERFORMANCE PROJECT IDENTIFICATON, shall be completed by the Offeror (the firm requesting the reference) prior to issuing the questionnaires. Offerors must identify only one Contract number in the Contract or Order Number field in Section A.
Section B, PAST PERFORMANCE REFERENCE INFORMATION, shall be completed AND signed by the Rater (only a Warranted Contracting Officer or Contracting Officer Representative with cognizance over the submitted project). For State/Local Contract submissions, a position equivalent to a Warranted Contracting Officer or Contracting Officer Representative shall complete and sign the Form.
Section C, RATINGS, shall be completed by the Rater.
No other format or additional proposal documentation will be considered in support of past performance data. The Offeror shall ensure that completed forms are submitted with its proposal.
INSTRUCTIONS TO RATER:
In order to assess potential vendors under the Department of Homeland Security (DHS) Tactical Communications (TacCom) II procurement, past performance information with respect to each Offeror is required. As the Executive Agent for this acquisition, the U.S. Customs and Border Protection, Office of Acquisition requests your support in obtaining past performance data for the project identified in Section A. Note, this form is only required from the Offeror when PPIRS information does not exist. As the Warranted Contracting Officer or Contracting Officer’s Representative with cognizance over the submitted project (or equivalent position for State/Local submissions), which the firm identified in Section A of this questionnaire, please complete Sections B and C. Please sign Section B using either ink or electronic signature. After completing Section B and C of the questionnaire, please return to the Offeror (the firm requesting the reference).
In addition to this questionnaire, you may receive a follow-up phone call to confirm or clarify information. This is a major DHS acquisition and will be a Multi-Agency Contract (MAC). Your input into this evaluation is greatly appreciated. For additional information regarding the definition and meaning of the rating areas, please consult the document titled “Guidance for the Contractor Performance Assessment Reporting Systems” available at www.cpars.gov and at this link: https://www.cpars.gov/pdfs/CPARS-Guidance.pdf.
The specific sections are: A3.27 for Quality of Service, A3.28 for Schedule, A3.29 for Cost Control, A3.30 for Management and A3.32 for Regulatory Compliance.
