CO/COR Certification Form – Federal
This form is to be completed by offerors submitting relevant experience project documentation from federal government agencies per RFP Section L.7.2.2 and L.7.2.3 (Item 9).
Instructions for Completion of this form:
INSTRUCTIONS TO OFFERORS
Offerors who indicate that a Relevant Experience Project Qualifying item or items on the Relevant Experience Qualification Checklist (Tab F5 (A), (B), and (C) of the TacCom II TC1/TC2 Proposal Workbook) is “Missing or Inconsistent Data Addressed by the CO,” must provide a narrative description of the missing or inconsistent data on this form and procure concurrence from the cognizant Contracting Officer (CO) or Contracting Officers Representative (COR) in accordance with RFP Section L.7.2.2.
Under the section titled Award ID: Identify the contract/order number or numbers to which the missing or inconsistent data applies and the relevant experience project number.
Under the section titled Description, identify the item number from the Relevant Experience Project Qualification Checklist and provide a brief description of the missing or inconsistent data. Expand the table as needed.
The description should be sufficiently detailed to explain the missing or inconsistent data. It should state what comprises the correct data for consideration by the TacCom II evaluators. The description should be sufficiently clear and complete so that the CO or COR fully understands the meaning of their concurrence. The onus is on the offeror to show the correlation between the relevant experience project attachments and the missing or inconsistent data that is certified by the cognizant CO/COR. Where the offeror fails to clearly make that correlation, the missing or inconsistent data may not be validated by the government and associated points deducted from offerors self-score.
Have the CO or COR sign and complete their section and return the signed document to you for inclusion in your response to the TacCom II RFP.
INSTRUCTIONS TO CERTIFYING COs/CORs:
In order to assess prospective offerors under the Department of Homeland Security (DHS) Tactical Communications (TacCom) II procurement, accurate past experience contract data is required. As the Executive Agent for this acquisition, the U.S. Customs and Border Protection, Office of Acquisition requests your support in obtaining this data for the project identified below. Note, this form is only required from the Offeror when the contractor has identified inconsistencies in supporting contract records (FPDS, CPARS, SOW, etc.) that are subject to validation in the TacCom II evaluation. As the Warranted Contracting Officer or Contracting Officer’s Representative with cognizance over the submitted project, please confirm whether the data presented by the contractor below is accurate. Please sign using either ink or electronic signature and return to the Offeror (the firm requesting the reference).
If you are certifying as a CO, complete the section titled CO Certification. If you are certifying as a COR, complete the section titled COR Certification and attach the email notification requested.
