Reference is requested to rate the offeror in each of the contract performance areas listed below using the following adjectival ratings.
EXCELLENT – Performance met contractual requirements and exceeded many of the customer’s benefit. Performance was accomplished with no to a few minor problems for which any required corrective actions taken were highly effective. (Risk level: Very low) GOOD – Performance met contractual requirements and exceeded some of the customer’s benefit. Peerformance was accomplished with some minor problems for which any required corrective actions taken were effective. (Risk level: Low) ACCEPTABLE –Performance met contractual requirements. Performance was accomplished with some minor problems for which any required corrective actions taken were satisfactory. (Risk level: Moderate) MARGINAL – Performance does not meet some contractual requirements. Performance was accomplished for serious problems for which any required corrective actions were marginally effective or not fully implemented. (Risk level: High) UNACCEPTABLE – Performance did not meet most contractual requirements. Performance was accomplished with serious problems for which any required corrective actions were ineffective or non-existent. (Risk level: Very high)
