Location Certification Request Form for Broward County

This is a Location Certification form issued by Broward County, Florida, for use in response to a Request for Proposal, Request for Qualification, or Request for Letter of Interest for Shuttle Bus and Other Transportation Services at Fort Lauderdale-Hollywood International Airport. The form enables vendors to certify their eligibility for local preference and location-based tiebreaker considerations under Broward County Code of Ordinances Section 1-74. Vendors must submit the completed certification form and all required supporting documentation by the solicitation end date to qualify for local preference, or within three business days of the County's written request. Failure to meet these submittal deadlines may render vendors ineligible for local preference or location points. The form is dated September 9, 2024.

The Location Certification form provides five certification options: Option 1 designates vendors as Local Businesses; Option 2 identifies vendors as both Local Businesses and Locally Based Businesses; Option 3 covers vendors classified as both Local Businesses and Locally Based Subsidiaries; Option 4 applies to joint ventures composed of Local Businesses, Locally Based Businesses, or Locally Based Subsidiaries; and Option 5 applies to vendors that do not qualify under any local preference category. Each option requires vendors to maintain a physical business address within Broward County for at least one year preceding the bid posting date, with supporting documentation including a local business tax receipt, proof of property ownership or right of use, and corporate organization documents where applicable. The form includes attestations that false or misleading information may result in suspension, debarment, contract termination, or civil and criminal penalties.
