About this file
This is a Drug-Free Workplace Certification Form required by Florida Statute 287.087 for the Hillsborough County Engineering and Operations Department's culvert replacement project. The form serves as a compliance document that vendors must complete and sign to certify adherence to Florida's drug-free workplace requirements. The certification applies to all employees engaged in providing commodities or contractual services under the bid, which in this case pertains to the replacement of three existing culverts located at 12026 Lenwood Lane, 111 1st Avenue NW, and 303 4th Street SE in Hillsborough County, Florida. The project is estimated at $1,958,866.12.
The Drug-Free Workplace Form requires vendors to certify six key compliance measures: publishing a workplace drug prohibition statement to employees, informing employees of drug abuse dangers and available support programs, distributing the prohibition statement to all relevant employees, notifying employees of their obligation to report convictions within five days, imposing sanctions or requiring rehabilitation program participation for violations, and maintaining good faith efforts to sustain a drug-free workplace. The form must be signed by an authorized company representative and submitted as part of the bid package. Completion of this certification is mandatory for bid consideration, and a six percent Small Business Enterprise (SBE) subcontractor participation goal has been established for this solicitation.
