About this file

This is a List of Subcontractors form for the Arizona Department of Transportation (ADOT) Invitation for Bid (IFB) BPM007067 for Used Automotive Fluid Collection and Removal Services. The procurement seeks a contractor to provide routine and emergency collection services for automotive fluids including motor oils, transmission fluids, antifreeze, contaminated fuels, sump materials, lubricants, oil filters, and oily rags across multiple ADOT, Department of Public Safety (DPS), and Arizona Department of Corrections (ADC) shop locations statewide. Collection and delivery services must occur Monday through Friday between 8:00 a.m. and 3:00 p.m., with non-routine services required within three business days and emergency services within 24-48 hours when possible. The solicitation was posted on September 17, 2025, with proposals due on October 9, 2025, through Arizona's eProcurement Portal (APP). The contract term is 12 months from award with potential extensions up to a maximum of 48 months through written amendments. A pre-offer conference was recommended but not mandatory.

The form requires bidders to declare whether they will subcontract portions of work to subcontractors or perform all work with their own personnel. If subcontracting is planned, bidders must identify all subcontractors with complete information including representative name, title, physical address, telephone number, email address, classification of work, license numbers, licensing agency, and APP supplier number. No change of identified subcontractors is permitted without prior written approval from the Procurement Officer. The contractor must provide comprehensive insurance coverage including commercial general liability ($2,000,000), business automobile liability ($1,000,000), workers' compensation, and pollution legal liability ($1,000,000 per occurrence). An administrative fee of 1.5 percent will be charged on all contract sales to cooperative purchasing members, with the fee increasing to 2 percent on January 1, 2025. The contract is available to all State of Arizona departments, agencies, boards, eligible universities, political subdivisions, and nonprofit educational or public health institutions participating in the State Purchasing Cooperative.
