STATEMENT OF WORK

REPAIR, Full Depth Roadway Patching Along Scott Drive, 375 CES; F7400

1 GENERAL
1.1 BACKGROUND Scott Air Force Base is one of the oldest Air Force bases in service, established in 1917 as Scott Field. Scott Drive was originally a two-lane Illinois State Route (two-lanes on East side) with ditch drainage. By 1944, the roadway had doubled in size when two-lane southbound travel lanes were added to the west, ultimately forming the boulevard. In 1947, Scott Field officially became Scott Air Force Base (SAFB). As SAFB continued to develop west of the current alignment, few improvements to storm water facilities to accommodate growth of SAFB were made. Similarly, little consideration was given to north-south non-motorized (bicycle and pedestrian) mobility. Thus, several deficiencies exist today including deteriorating pavement, substandard drainage facilities, inconsistent typical section, and lack of bicycle and pedestrian facilities.
1.2 OBJECTIVES This project is to complete full depth patching along Scott Drive to repair sections of pavement in poor condition.
1.3 STANDARDS, REGULATIONS, AND CODES The work associated with this project shall adhere to all applicable standards, regulations, and codes, in effect as of this date of this statement of work (SOW), as detailed in the Scott AFB MAPC General Requirements (12 SEP 2019) - Section 01010 General Requirements.

2 PROJECT ADMINISTRATION
2.1 SITE VISIT Contractor is responsible to verify all measurements and conditions pre-bid. The Government intends to only hold one official site visit. Requests for additional site access must go through the 375th Contracting Squadron (375 CONS) point of contact (POC) listed in the Request for Proposal (RFP) letter. Any additional site access will be made available to all Contractors solicited for this project.
2.2 MEETINGS
2.2.1 Kick-Off Meeting/Project Schedule – Kick-off meeting to occur within one week of NTP.
Overall project schedule with milestone markers and 2-week look-ahead to be completed at kick-off meeting.
2.2.2 Weekly Meeting - Contractor shall coordinate with the 375th Contracting Squadron (375 CONS) on weekly meetings once the construction phase begins. This meeting shall include the review of an updated 2-week look-ahead schedule. Contractor shall provide meeting minutes within 24 hours of each meeting and allow the Government up to 24 hours to accept or request changes to the minutes.
3 TASKS AND TECHNICAL REQUIREMENTS
3.1 BASE BID
3.1.1 Contractor shall provide services to support any deliverables listed in Section 4 (Deliverables) of this SOW and all attachments.
3.1.2 Contractor shall remove and dispose of approximately 6600 square feet of the roadway from Scott Drive as shown in ATCH 1.
3.1.2.1 Contractor shall remove existing roadway pavement in accordance with Section 440 and Section 442 of the IDOT Standard Specifications. Transverse joint spacing shall be considered when determining the patch size.
3.1.2.2 Curb & gutter adjacent to the repair area shall be removed along with the roadway pavement.
3.1.2.3 There are a total of 4 patches to be completed. 3 patches are located in the south bound lane, the lengths of these patches are 18’, 40’, and 34’ from north to south. 1 patch with a length of 120’ is located in the northbound lane.
3.1.2.4 Boring log results are shown in ATCH 3 depicting the existing typical pavement sections.
3.1.2.5 A utility map of the area has been included in ATCH 4.
3.1.3 The contractor shall excavate the existing subgrade in accordance with section 202 of the IDOT Standard Specifications to allow for 12” of compacted CA6 base course and 8” of concrete pavement.
3.1.3.1 Any existing aggregate and earth material to be excavated shall be removed from the base.
3.1.3.2 Prior environmental test results have been provided in ATCH 2. These results indicate that the soil underneath the pavement in the arial photo given is contaminated and will not be able to be removed as clean fill.
3.1.3.3 Contaminated soil shall be tested for contaminates prior to removal and disposal at the appropriate landfill for the contaminated material
3.1.3.4 Areas shown to not be contaminated can be handled as Certified Clean Fill Per 35 IAC Part 1100. The contractor shall keep clean material separate from contaminated material.
3.1.3.5 Contaminated loads of material that are being removed from base shall have a manifest signed by an approved Base Environmental representative.
3.1.3.6 The contractor shall protect all stormwater inlets within 50 yards of the work area from debris infiltration in accordance with Scott Base Wide SWPPP (ATCH 5).
3.1.3.7 The Contractor shall be responsible for implementing storm water BMPs to prevent prohibited discharges from entering storm drains during all phases of construction in accordance with Scott Base Wide SWPPP (ATCH 5).
3.1.4 In areas without failed subgrade, the contractor shall install 12” of aggregate base course in accordance with Section 351 of the IDOT Standard Specifications. The gradation for the aggregate shall be CA-6. The aggregate base course shall extend a minimum of 1’ behind the back of curb.
3.1.5 The contractor shall construct new curb and gutter in accordance with Section 606 of the IDOT Standard Specifications and IDOT Highway Standard 606001. The curb and gutter shall be constructed separate from the adjacent pavement and shall be tied to the adjacent pavement and doweled into adjacent concrete curb and gutter.
3.1.6 The contractor shall construct new roadway pavement in accordance with Section 420 of the IDOT Standard Specifications and IDOT Highway Standard 420001.
3.1.6.1 The new pavement shall be 8” thick Class PV, Type III concrete with a Type B finish.
3.1.6.2 The spacing of the transverse joints shall be between 10’ and 15’.
3.1.6.3 Contractor shall complete work on one direction of traffic at a time. Once re-opened, contractor can then close the opposite direction.
3.1.6.4 All reinforcing steel and dowel bars shall be epoxy coated.
3.1.6.5 Accelerating admixtures cannot be used. Water reducing and/or retarding admixtures may be added to the concrete at the project site, but water shall not be added to the concrete mixture after it leaves the concrete plant.
3.1.6.6 Provide the following documentation related to concrete construction. All tests must have been run within the last 6 months, unless indicated otherwise:
(a) Mixture proportion including all admixtures.
(b) Gradation and quarry for all aggregates
(c) ASTM C 1260 for each aggregate. (within last 36 months)
(d) Sulfate soundness for coarse aggregate.
(e) Mill certificates for all cementitious materials.
(f) Certificate for epoxy coating of all reinforcement
(g) Buy American certificates for all steel
(h) Compressive strength results (within last 12 months)
3.1.7 Traffic control on Scott Drive will require the contractor to work closely with the project manager, contracting officer, and security forces.
3.1.7.1 Traffic control and phasing shall be in accordance with Section 701 of the IDOT Standard Specifications and the applicable IDOT Highway Standards for traffic control and protection to minimize the impact to the consistent traffic.
3.1.7.2 This project will cause a large disturbance to traffic throughout the workday, especially during heavy traffic hours in the morning and afternoon. The contractor shall coordinate all road closures and changes to road closures at least 2 weeks prior to the closure.
3.1.7.3 The contractor shall provide all traffic control devices for this project. This shall include traffic signs, traffic cones, traffic barricades, and electronic message boards to alert drivers of the ongoing work.
3.1.7.4 This project will have to be constructed in at least 2 different phases. The contractor may suggest their own phasing plan for approval, this may be more than 2 phases if it proves to be beneficial to the completion of this project.
3.1.7.5 Contractor shall include a traffic control plan with proposal for review. An example traffic control plan is shown in ATCH 6, note this is an example for reference and not specific for this project. Contractor should create and submit full traffic control plan for review.
3.1.8 The contractor shall apply new paint markings to the new pavement before the completion of the project. Paint markings shall be in accordance with SDDCTEA 55-17.
3.1.9 Disturbed grassy area in the work area shall be regraded and seeded by the contractor prior to the end of the project in accordance with Section 250 and Section 251.
3.1.9.1 Contractor shall seed, fertilize, and cover any disturbed grassy areas.
3.1.9.2 The contractor shall use a 12-12-12 fertilizer on the newly placed seed and complete 2 waterings of the area saturating the soil within the first 5 days.
3.1.9.3 Once placed, the contractor shall cover the newly placed seed with straw mulch.
3.1.10 Any damage to the surrounding area caused by the contractor shall be repaired by the contractor.
3.2 An alternate method can be proposed for any of the above tasks and will be reviewed. Any alternate methods or deviations from the requested work shall be called out or highlighted in the Contractor’s proposal in a manner that is clearly identifiable. Any approved alternate methods will result in a SOW modification prior to the commencement of work. The Government will assume any additional design or construction costs associated with the approved alternatives but will not assume any costs associated with the Contractor processing this required modification.
4 DELIVERABLES
Contractor shall provide deliverables as detailed in Scott AFB MAPC General Requirements (12 SEP 2019) - Section 01010 General Requirements. In addition to the items detailed in Scott AFB MAPC General Requirements (12 SEP 2019) - Section 01010 General Requirements, the following schedule of items are required for submittal as a part of this project:
• The contractor shall provide material submittals for all materials that are to be used on this project. This includes but is not limited to concrete, reinforcement steel, aggregate base course, backer rod, joint sealant, marking paint, glass beads, grass seed, and fertilizer.
• The contractor shall submit traffic control plans for approval and public notification prior to all traffic control changes. This plan shall include routes, signage, barricades, and pavement markings.
5 SCHEDULE
5.1 The period of performance (PoP) for this project is approximately 65 calendar days from the date that Notice to Proceed (NTP) is issued. All work must be scheduled to be complete within this timeframe. Any modifications or extensions must be requested through the 375 CES and the Contracting Officer (CO) for review and discussion.
5.2 Prior to beginning construction, Contractor shall provide to the CO and 375 CES construction milestones in sufficient detail to accurately identify all phases/disciplines of work to be performed as detailed below:
5.2.1 Contractor shall provide detailed progress schedule including completion percentages for each line item.
5.2.2 Contractor shall provide general construction schedule, showing, at a minimum, the critical path items.
5.3 Contractor will be required to provide 2-week look ahead at each meeting described above.
5.4 Construction may be authorized by the Contracting Officer upon acceptance and approval of the schedule and equipment/material submittal approvals.
6 GENERAL REQUIREMENT ADDENDUM
Contractor shall adhere to Scott AFB MAPC General Requirements (12 SEP 2019) - Section 01010 General Requirements unless otherwise noted below. Contractor shall comply with requirements detailed in SOW ATCH 5 – Contractor Requirements.
7 PROJECT MANAGER
The 375 CES Project Manager (PM) for this project is J. Andrew Swisher at (618) 256-3866 or jonathon.swisher.2@us.af.mil. The PM is the point of contact for all matters of design and, unless a construction inspector is designated, construction. For matters concerning terms and conditions of the contract with the Government, the 375 CONS designated official is the point of contact.
