Mandatory submission requirements include completed vendor forms, certifications, key position resumes, project experience documentation, and technical/business proposals submitted separately through the DHSS Bonfire Procurement Portal.

The RFP does not specify set-asides for disadvantaged enterprises or small business designations, though bidders must complete a Supplier Diversity plan statement and identify Tier II subcontractors as part of mandatory submission requirements. Vendors must maintain Commercial General Liability insurance of $1,000,000 per occurrence/$3,000,000 aggregate and either Medical/Professional Liability, Errors and Omissions, or Product Liability coverage at the same limits; additionally, vendors must carry cyber liability insurance if unable to encrypt personally identifiable information at rest. All data must remain within the United States, and key project personnel, including the Project Manager, must maintain on-site presence at the Biggs Data Center in New Castle, Delaware, throughout the project phase. The RFP documentation does not identify incumbent contractors or provide specific budget ranges; however, cost proposals are structured with separate firm fixed pricing for project lifecycle phases (Phase 1-3) and itemized annual operations and support costs for the post-deployment period (Phase 4) across ten years, with Phase 5 weighted at 40 percent of total project implementation costs and all milestones subject to 20 percent holdbacks except the final phase. The State of Delaware reserves the right to award to multiple vendors and to negotiate simultaneously with more than one vendor.

The Bureau of Cancer Prevention and Control (BCPC) and Bureau of Chronic Disease (BCD) are ideally positioned in the Health Promotion and Disease Prevention (HPDP) Section of the Delaware Division of Public Health (DPH). The BCPC programs provide administrative staff support for the Delaware Cancer Consortium (DCC) and affiliated committees. The COVID-19 pandemic unveiled the depth of health inequities in the U.S. Low-income communities and Delaware's most vulnerable populations experience a disproportionate burden of coronavirus in terms of both health outcomes and economic impact. Through this data modernization project, the BCPC and BCD aims to improve data accuracy and utilize its cancer and chronic disease data to inform program activities and to conduct analyses to inform DPH leadership, healthcare providers, and other stakeholders. Bidders of this RFP must propose equity focused data collection, monitoring, and visualization strategies to support the achievement of the following cancer prevention and control, and chronic disease management goals: eliminate preventable cancers, ensure all people get the right screening at the right time for the best outcome and support cancer survivors in a manner that allows them to live longer heathier lives, track and manage patient information for the Chronic Disease Self-Management Program, Healthy Behavior Support Service Programs, WISEWOMAN Program, and the Emergency Medical Diabetes Fund. The BCPC houses three components of CDC's National Cancer Prevention and Control Program grants within one bureau: o National Program of Cancer Registries (NPCR)/Delaware Cancer Registry (DCR) o National Breast & Cervical Cancer Early Detection Program (NBCCEDP)/Screening for Life (SFL) o National Comprehensive Cancer Control Program (NCCCP)/Comprehensive Cancer Control Program (CCCP) The BCPC also houses two primarily state-funded programs: o Health Care Connection (HCC) o Delaware Cancer Treatment Program (DCTP) The Bureau of Chronic Disease (BCD) implements the following programs: o Health Coaching through the Division of Public Health Self-Management Programs (SMP) o Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-BPSM) o CDC's The Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) o Delaware Emergency Medical Diabetes Fund . Project Goals DATA MANAGEMENT: This front-end data collection system is used to support DPH's response to statewide strategies aimed at mitigating the impact of chronic disease risk factors affiliated with cancer, hypertension, diabetes asthma and other chronic conditions. This database works simultaneously with Smartsheet's data collection tool to collect specific points of data relating to all BCPC and BCD programs. The goal of this RFP is to integrate these two data collection and management tools. This will align federal grant requirements, internal state operations with external clinical operations with SFL clients, comprehensive cancer program providers and health systems which will allow for ease of data collection, production of federal/state reports and establish a more robust centralized hub to systematically support decision making. BCPC and BCD program operations under this RFP will require cross-collaborations between all programs and successful applications will propose evidence, practice, or research-based strategies that support expanding inclusivity and contribute to more racially equitable outcomes to ensure sustained interventions that achieve Delaware's goals for Quadruple Aim - improving the health of Delawareans, enhancing the patient experience of care, reducing health care costs and improving provider satisfaction. This RFP program is DPH's opportunity to strategize about how to work across sectors, address the long-term health and social needs of a community and remove structural barriers to health for socially vulnerable populations in Delaware.
